Background and Objective Mechanisms underpinning symptoms in non‐traumatic neck pain (NTNP) and whiplash‐associated disorder (WAD) are not comprehensively understood. There is emerging evidence of systemic inflammation in musculoskeletal pain conditions, including neck and back pain. The aim of this systematic review was to determine if raised blood inflammatory markers are associated with neck pain. Databases and Data Treatment MEDLINE, EMBASE, Cochrane Library, CINAHL and Web of Science databases were searched. Two independent reviewers identified studies for inclusion and extracted data. Meta‐analysis was performed by random effects model to calculate standard mean differences (SMDs). Risk of bias of individual studies was assessed using the Newcastle–Ottawa Scale. Overall quality of evidence from meta‐analysis was assessed by Grades of Recommendation, Assessment, Development and Evaluation approach. Results In total, 10 studies were included comprising 706 participants. Three studies provided data for acute WAD, two for chronic WAD, four for chronic NTNP and one for chronic mixed WAD and NTNP. Meta‐analysis indicated increased interleukin 1β (SMD: 0.84 [95% CI 0.24, 1.44], p = .01, I2 = 59%) and tumour necrosis factor α (SMD: 0.59 [0.09, 1.09], p = .02, I2 = 45%) in chronic neck pain compared to controls, but no increase in monocyte chemoattractant protein‐1. Some inflammatory markers were associated with clinical variables (including pain intensity and disability). Quality of evidence was mostly low due to small samples and high heterogeneity. Conclusions Findings imply that raised blood inflammatory markers are present in chronic neck pain, which may represent an ongoing inflammatory process in this population. Significance This systematic review advances our understanding of neck pain pathophysiology by demonstrating the presence of systemic inflammation in chronic neck pain, in the form of raised IL‐1β and TNFα. Further, numerous inflammatory markers were associated with clinical variables, including pain intensity, disability and hyperalgesia. These findings imply that systemic inflammation may contribute to mechanisms underlying neck pain.

IntroductionJoint mobilisation and manipulation often results in immediate pain relief in people with neck pain. However, the biological mechanisms behind pain relief are largely unknown. There is preliminary evidence that joint mobilisation and manipulation lessens the upregulated neuroimmune responses in people with persistent neck pain.Methods and analysisThis study protocol describes a randomised placebo-controlled trial to investigate whether joint mobilisation and manipulation influence neuroimmune responses in people with persistent neck pain. People with persistent neck pain (N=100) will be allocated, in a randomised and concealed manner, to the experimental or control group (ratio 3:1). Short-term (ie, baseline, immediately after and 2 hours after the intervention) neuroimmune responses will be assessed, such as inflammatory marker concentration following in vitro stimulation of whole blood cells, systemic inflammatory marker concentrations directly from blood samples, phenotypic analysis of peripheral blood mononuclear cells and serum cortisol. Participants assigned to the experimental group (N=75) will receive cervical mobilisations targeting the painful and/or restricted cervical segments and a distraction manipulation of the cervicothoracic junction. Participants assigned to the control group (N=25) will receive a placebo mobilisation and placebo manipulation. Using linear mixed models, the short-term neuroimmune responses will be compared (1) between people in the experimental and control group and (2) within the experimental group, between people who experience a good outcome and those with a poor outcome. Furthermore, the association between the short-term neuroimmune responses and pain relief following joint mobilisation and manipulation will be tested in the experimental group.Ethics and disseminationThis trial is approved by the Medical Ethics Committee of Amsterdam University Medical Centre, location VUmc (Approval number: 2018.181).Trial registration numberNL6575 (trialregister.nl

Parkinson’s disease is a degenerative disease of the central nervous system, and it occurs in middle-aged and elderly people. Studies have shown that both the clinical symptoms and neuropathological evidence of Parkinson’s disease suggest that Parkinson’s disease may originate in the gut. Intestinal flora homeostasis plays an important role in maintaining normal functions of the brain and nervous system. It participates in changes in cellular flora through oxidative stress, inflammatory response, and immune response during metabolism. Intestinal flora disorders are closely related to the onset of neurological diseases such as Parkinson’s disease (PD). In order to better understand the relationship between intestinal flora and Parkinson’s disease, this article studies the correlation between PD rat models and intestinal flora, and analyzes the possible relationship between them. The 6-OHDA PD rat model is currently a better model preparation method, which is widely used in PD research. The experimental results show that using Fe3O4 nanoparticle technology to detect intestinal flora disorders in PD patients, and the role of intestinal flora disorders in Parkinson’s disease may include affecting inflammatory response and oxidative stress, α-synuclein Protein (α-syn), these modes of action are not independent, there are complex and synergistic effects, and the molecular simulation mechanism may play a key role in these effects. There is a certain relationship between intestinal flora and Parkinson’s disease, but the specific mechanism is not clear, and further research is needed to provide more directions for the early diagnosis and early treatment of PD.
