Effect of Testosterone Replacement on Trabecular Architecture in Hypogonadal Men

Abstract: ABSTRACT:We evaluated the effect of testosterone treatment on trabecular architecture by µMRI in 10 untreated severely hypogonadal men. After 2 years, µMRI parameters of trabecular connectivity improved significantly, suggesting the possibility that testosterone improves trabecular architecture.Introduction: Osteoporosis, characterized by low BMD and diminished bone quality, is a significant public health problem in men. Hypogonadal men have decreased BMD and deteriorated trabecular architecture compared with … Show more

“…A further potential motivation is the study of the structural manifestations of metabolic bone disease such as renal osteodystropy (166). Evidence for these developments is the rapid growth of the clinical literature (8,83,84,127,145,165,(167)(168)(169)(170)(171)(172) and also, to a lesser extent, the increased use of CT to characterize TB (14)(15)(16)(173)(174)(175)(176)(177).…”

“…Several studies reported during the past 3 years examine the structural implications of antiresorptive treatment (84,145,164,172). Benito et al(172) studied the effect of testosterone treatment on architecture in the distal tibial metaphysis of 10 severely testosterone-deficient hypogonadal men and their age-matched controls by means of the MRI-based virtual bone biopsy (19).…”

“…BV/TV, trabecular separation (Tb.Sp), trabecular thickness (Tb.Th) and trabecular number (Tb.N)], digital topologic analysis of μMRIs generates estimates of trabecular connectivity and the conversion of plates to rods [62]. Recent studies demonstrate that these measures improve fracture prediction and are sensitive to structure changes induced by testosterone replacement in hypogonadal men [63] and hormone replacement therapy in women [64].…”

“…Quantitative musculoskeletal imaging has been employed at standard field strengths to assist in the clinical diagnosis of diseases such as osteoarthritis (OA) and osteoporosis (OP), to monitor the progression of diseases and to evaluate response to treatment with structure/disease-modifying drugs [4][5][6][7]. The measurement of cartilage morphology may provide a biomarker for evaluating the long-term progression of OA.…”
