Complementary and alternative medicine (CAM) is often implemented in kidney stone patients. It consists of preparations including different ingredients, such as herbs, probiotics, and vitamins, often together with alkali, that are classified within the dietary supplementation category. The majority of dietary supplements claiming to treat or prevent kidney stones contain ingredients with conflicting or no scientific evidence to support their claims. Clinicians should advise stone formers that the effects of most supplements are unknown or unstudied in humans and that the absence of evidence does not imply absence of potential harm. Unfortunately, the CAM preparation consists of a mix of different molecules, often including alkali, with different potential mechanisms of action and, even when favorable results are reported, the role of the single molecules cannot be assessed. Despite all these concerns, CAM products remain quite popular among kidney stone patients. The scarce knowledge in this field prevents one from recommending CAM products in daily clinical practice; only a weak suggestion for their use in kidney stone patients may be reasonable.

A constant increase in the features of metabolic syndrome leading to serious complications including neuropathy, retinopathy and nephropathy has resulted in worldwide epidemic of Type 2 diabetes mellitus or insulin resistance. Most of the available antidiabetic drugs were developed in the absence of defined molecular targets or a clear understanding of disease pathogenesis. Moreover, the available drugs suffer from one or the other side effects. Emerging knowledge of key physiologic mechanisms related to probable causes of hepatic and muscle insulin resistance on glucose metabolism has led to a number of new emerging molecular drug targets. Type 2 diabetes mellitus is a major contributor of deaths in India and worldwide, there is a urgent need to develop new drugs that will be helpful to diminish the overall diabetic load of world population. The present review provides the description of existing antidiabetic drugs and their respective molecular targets. There is a information regarding the antidiabetic targets that include receptors and enzymes that enhance glucose-stimulated insulin secretion, suppress hepatic glucose production, increase skeletal muscle glucose transport and utilization, increase insulin sensitivity and intracellular insulin signaling with reduction in circulating and intracellular lipids. It also emphasizes on the need for developing novel antidiabetic drugs having lower side effects and more efficacy.

Background and Objectives: Amomum subulatum (AS) is used to improve cardiac health in traditional medicine practice. The present study evaluates the pharmacological effect of AS aqueous extract on blood pressure in Normotensive (NR) and Spontaneously Hypertensive Rats (SHR). Materials and Methods: Blood pressure, Heart Rate (HR) and Heart Rate Variability (HRV), was recorded in catheterized Sprague-Dawley rats before and after AS intravenous administration by using Mikro-Tip Pressure-Volume System (MPVS), PowerLab. The receptor activity was assessed by using the drugs Acetylcholine (ACh) and Atropine (Atr). Results: Preliminary phytochemistry of AS suggests that it contains tannins, flavonoids and saponins. Mean Arterial Pressure (MAP) was found to decrease significantly in NR and SHR as compared with the control. The lowest dose (1 mg kgG 1 ) produced the least (16%) while 30 mg kgG 1 caused the maximum reduction (40%) in MAP. Electrocardiograph analysis revealed a significant increase in RR interval (decreased heart rate), time-domain Standard Deviation of Interbeat Interval (SDNN) and the Root Mean Square of the Successive Differences (RMSSD) and High-frequency Domain (HF%) parameters and a decrease in the Low-Frequency (LF) range, suggesting the activation and involvement of the parasympathetic limb. It was also observed that the cardiovascular effects of AS were comparable to Acetylcholine (ACh) and both were completely blocked by Atropine (1 µg kgG 1 ). Conclusion: The obtained results suggest that AS has a hypotensive effect, with an impact on the HRV of NR and SHR. Amomum subulatum might cause an augmented effect on the cholinergic limb of the Autonomic Nervous System (ANS) and decrease the blood pressure and heart rate significantly.
