Effectiveness of App-Based Cognitive Screening for Dementia by Lay Health Workers in Low Resource Settings. A Validation and Feasibility Study in Rural Tanzania

Abstract: Background: The majority of people with dementia live in low-and middle-income countries (LMICs). In sub-Saharan Africa (SSA) human-resource shortages in mental health and geriatric medicine are well recognized. Use of technological solutions may improve access to diagnosis. We aimed to assess the diagnostic accuracy of a brief dementia screening mobile application (app) for non-specialist workers in rural Tanzania against blinded gold-standard diagnosis of DSM-5 dementia. The app includes 2 previously-validat… Show more

“…The IDEA‐IADL App‐Based Cognitive Screening instrument mobile application has now been developed and validated among non‐specialist rural community workers. The AUROC was 0.78 with good sensitivity and fair specificity 158 . In Central Africa, the Central African Daily Functioning Interference Scale (CA‐DFI), a 10‐item scale, has also been developed with psychometric properties supporting the reliability and internal validity of the CA‐DFI scale as a promising tool for functional assessment in the elderly for the diagnosis of dementia in Central Africa 159 …”

“…It is clear, however, that smartphones are more accessible and cheaper. Presently, less information is available about applications on these devices for measuring cognition in children, smartphone-based cognitive assessments for other populations, such as adults with dementia, have recently been developed [ 48 , 49 ]. It is likely that this review will need updating considering the fast-moving field and will need to include a focus on smartphone-based tools once data on their validity are available.…”

“…This allows quality control, monitoring of data, and access to prompt support and clarification of queries from data collectors. Tablet‐based administration of cognitive screening was an accurate and feasible method of assessment in this setting 29 . Currently, accurate screening measures for identification of frank dementia in Tanzania are available, but there is an ongoing need for measures of early cognitive decline and sensitive measures of cognitive change.…”
