Effects of botulinum toxin type A on intracortical inhibition in patients with dystonia

Abstract: To find out whether botulinum toxin alters the excitability of cortical motor areas, we studied intracortical inhibition with transcranial magnetic stimulation in patients with upper limb dystonia before, 1 month after, and 3 months after the injection of botulinum toxin type A in the affected muscles. Eleven normal subjects and 12 patients with dystonia involving the upper limbs (7 with generalized dystonia, 2 with segmental dystonia, and 3 with focal dystonia) were studied. Patients were assessed clinically … Show more

“…For example, changes have been described at the level of spinal cord circuitry (Priori et al, 1995;Wohlfarth et al, 2001). An alteration of cortical excitability has also been reported in dystonic patients treated intramuscularly with BoNT/A (Kanovsky et al, 1998;Gilio et al, 2000).…”

“…A similar study was conducted in patients with upper limb dystonia to investigate the before and after effects of BT-A on intracortical inhibitory circuits of the primary motor cortex [95]. Before treatment, patients showed intracortical inhibition to be reduced which returned to values seen in normal subjects one month after the injection.…”

“…However, after three months of BT-A injections values of intracortical inhibition dropped again to pretreatment levels. This study suggests that BT-A can transiently modify the excitability of the motor cortical areas by reorganizing inhibitory and excitatory intracortical circuits [95]. In patients with blepharospasm, BT-A has also been shown to reduce the abnormally enhanced plasticity response of the trigeminal blink reflex [96].…”

“…Более того, у взрослых пациентов с цервикальной дистонией введение БТА в мышцы шеи изменяло возбудимость моторных нейронов коры головного мозга [79], что, однако, не наблюдалось при инъекциях БТА в мышцы верхних конечностей [80]. Данные закономерности могут изменяться в ситуации с пациентами младшего воз-раста за счет меньших размеров всех анатомических структур и длины проводящих путей.…”
