Efficacy of ECT in Depression: A Meta-Analytic Review

Abstract: This study analyzed the efficacy of electroconvulsive therapy (ECT) in depression by means a meta-analytic review of randomized controlled trials that compared ECT with simulated ECT or placebo or antidepressant drugs and by a complementary meta-analytic review of nonrandomized controlled trials that compared ECT with antidepressants drugs. The review revealed a significant superiority of ECT in all comparisons: ECT versus simulated ECT, ECT versus placebo, ECT versus antidepressants in general, ECT versus TCA… Show more

“…Recently, we discovered that there was less-enhanced, albeit still significant, autophagy signaling in the hippocampus after antidepressive (Pagnin et al, 2004) multiple ECS treatments (Otabe et al, 2014) without neuronal cell death (Devanand et al, 1994). Since ECS has been shown to downregulate the number of specific neurotransmitters receptors (Pandey et al, 1979;Nakata et al, 1985), we postulated that the enhanced autophagy signaling could be related to neuronal plasticity.…”

“…Repeated ECS induces sprouting of mossy fibers [89-91], but antidepressant drug treatments do not [91]. The ability of ECS to induce mossy fiber sprouting might be related to the superiority of the electroconvulsive therapy in effectiveness against depression as compared with antidepressant drugs [92]. However, the clinical significance of the ECS-induced sprouting remains to be elucidated.…”

“…Electroconvulsive therapy (ECT) has been used as a treatment for psychiatric disorders since the 1930s and its effectiveness in particular in severe depression has been largely investigated (Group 2003;Pagnin et al 2004). For a long time, it has been the only established therapy for patients with severe or treatment-resistant depression (TRD).…”
