Efficacy of magnetic resonance imaging in the diagnosis of perianal hidradenitis suppurativa, complicated by anal fistulae: A report of two cases and review of the literature

Abstract: HighlightsPerianal hidradenitis suppurativa often recurs due to incomplete resection of anal fistulae.MRI is more accurate for detecting anal fistulae than CT or fistulogram. We report the usefulness of preoperative MRI in performing complete resection.

“…HS pathology, like many disease states, alters the water content of the tissue as a result of the increased inflammation. These areas tend to be composed of subcutaneous sinus tract networks which are easily visible on MRI due to the high contrast resolution . MRI has been primarily utilized for severe, anogenital HS lesions and is typically associated with other pathologies like IBD .…”

“…Most recently, and in conjunction with MIT, MRI has been effectively utilized in the pre‐operative assessment of a characteristically severe HS lesion . An MR image of HS is present in Figure . Owing to the high cost and limited ability to detect small, superficial lesions, MR imaging is primarily utilized in pre‐surgical planning of extensive cases of HS.…”

“…60 The postcontrast sequences demonstrate extensive complex branching perianal, perirectal, and subcutaneous sinus tracts and fistulas. 62 In addition, there are often multiple small rim-enhancing abscesses in the subcutaneous soft tissues of the perineum, ischioanal region, and medial thigh. 59,60 In some cases, the disease extends into the pelvis, with presacral fistulas and abscesses.…”
