Increased awareness of periampullary & pancreatic head cancers, and the accompanying improved outcomes following pancreatoduodenectomy (PD), has possibly led to an increase in patients seeking treatment for the same. While there has definitely been a reduction in morbidity rates following PD in the last few decades, this decline has not mirrored the drastic fall in mortality. Amongst the foremost in the factors responsible for this reduction in mortality is the standardization of surgical technique and development of dedicated teams to manage all aspects of this demanding procedure. This review intends to provide the reader with an overview of major complications following this major surgery and measures to prevent them based on the authors' experience.

Systematic Review and Meta-Analysis of Somatostatin Analogues in the Prevention of Postoperative Complication after Pancreaticoduodenectomy

Background: The use of somatostatin analogues (SAs) following pancreaticoduodenectomy (PD) is controversial. Method: Literature databases were searched systematically for relevant articles. A meta-analysis of all randomized controlled trials (RCTs) evaluating prophylactic SAs in PD was performed. Results: Fifteen RCTs involving 1,352 patients were included. There was a towards reduced incidences of pancreatic fistulas (p = 0.26), clinically significant pancreatic fistulas (p = 0.08), and bleeding (p = 0.05) in prophylactic SAs group. In subgroup analyses, prophylactic somatostatin significantly reduced the incidence of pancreatic fistulas (p = 0.02), with a nonsignificant trend toward reduced incidence of clinically significantly pancreatic fistulas (p = 0.06). Pasireotide significantly reduced the incidence of clinically significantly pancreatic fistulas (p = 0.03). Octreotide had no influence on the incidence of pancreatic fistulas. Conclusion: The current best evidence suggests prophylactic treatment with somatostatin or pasireotide has a potential role in reducing the incidence of pancreatic fistulas, while octreotide had no influence on the incidence of pancreatic fistulas. High-quality RCTs assessing the role of somatostatin and pasireotide are required for further verification.

A szomatosztatin az emésztőrendszerre sokrétűen hat. Gátolja az emésztőrendszeri hormonok felszabadulását, a gyo mor, a hasnyálmirigy és az epe exokrin működését, csökkenti a motilitást és befolyásolja egyes tápanyagok felszívódását is. E sokirányú hatás alapján fokozott várakozás előzte meg a szomatosztatinkezelés eredményessé-gét különböző emésztőrendszeri betegségekben. A szomatosztatinkezelés realitását az elhúzódó hatású analógok (octreotid, lanreotid) kialakítása teremtette meg. Az elmúlt negyed században kiterjedt vizsgálatok tisztázták a szomatosztatinanalógok eredményességét és helyét különböző emésztőrendszeri betegségek kezelésében. A tanulmány az eredmények rövid összefoglalására és értékelésére tesz kísérletet. A kezelés az emésztőrendszer számos kóros álla-potában hatékony, a klinikai gyakorlat részévé azonban csak a neuroendokrin daganatok elsődleges konzervatív, valamint a nyelőcsővisszér-vérzések és a hasnyálmirigysipolyok kiegészítő kezelésében vált. Based on these diverse effects there was an increased expectation towards the success of somatostatin therapy in various gastrointestinal disorders. The preconditions for somatostatin treatment was created by the development of long acting somatostatin analogues (octreotide, lanreotide). During the last twenty-fi ve years large trials clarifi ed the role of somatostatin analogues in the treatment of various gastrointestinal diseases. This study summarizes shortly these results. Somatostatin analogue treatment could be effective in various pathological conditions of the gastrointestinal tract, however, this therapeutic modality became a part of the clinical routine only in neuroendocrine tumours and adjuvant treatment of oesophageal variceal bleeding and pancreatic fi stulas.
