Electroanatomic Mapping Guided Cryoablation of Mahaim Pathways in Children with Limited Fluoroscopy Exposure

Abstract: Cryoablation can be used as a reliable and effective alternative to RFA in the treatment of Mahaim accessory conduction pathways in children. Prospective comparative studies are necessary in order to further evaluate the long-term efficacy of this method.

“…In the posteroseptal region, APs often take an epicardial instead of endocardial course which impedes successful ablation . The use of nonfluoroscopic 3‐D mapping in combination with nonirrigated radiofrequency ablation (RFA) of right‐sided APs has been increasingly reported in children and adults . Few data exist about the use of irrigated‐tip catheters (ITCs) for right‐sided APs in children and adolescents, and no systematic data are available about the combination of ITC with a 3‐D mapping system in this age group.…”

“…These fluoroless procedures are exceedingly significant in specific patient subpopulations who are at a higher risk of adverse effects from radiation exposure such as pregnant women, patients with obesity, and pediatric patients. 4,7,8,[10][11][12][13][14][15] In this review, we sought to describe how fluoroless procedures are carried out and highlight the new tools available to EP operators in this area of practice. In addition, we present the most recent data on fluoroless procedure outcomes.…”
