Elevated 68Ga-FAPI Uptake by Primary Benign Intraosseous Meningioma

Abstract: Intraosseous meningioma is an extremely rare benign tumor. We present the 68Ga–fibroblast activation protein inhibitor (FAPI) PET/CT findings of primary intraosseous meningioma in a 71-year-old woman. 68Ga-FAPI PET/CT revealed an intraosseous mass in the right parietal bone with increased FAPI activity. Primary skull malignancy was suspected. However, pathological examination after resection of the mass in the right parietal bone confirmed the diagnosis of benign meningioma (WHO I). A final diagnosis of benign… Show more

“…[5][6][7][8][9][10] A recent case has also reported mild FAPI activity in a benign intraosseous meningioma. 11 Also, in this case of atypical meningioma, FAPI activity was mild despite the malignant characteristics of the tumors. On the other hand, SSTR-2 imaging showed significant uptake compared with heterogeneous mild FAPI activity, which could be related to the heterogeneity of the tumoral lesions and the associated stroma.…”

“…FAPI is a novel tracer with theranostic properties 4 used for the diagnostic imaging of several malignant tumors 5–10 . “…[4][5][6] Fibroblast activation protein inhibitor-based radiotracers are emerging as excellent agents to target fibrotic and tumor-associated stromal tissues for theranostic applications. 7,8 A recent case report has shown FAPI activity in benign intraosseous meningioma and role of 68 Ga-DOTATOC and 18 F-FDG. 8,9 Another case report has shown FAPI activity in a patient with multiple meningiomas, showing somatostatin receptor expression in meningioma lesions.…”

“…However, FAP expression is not cancer-specific; it is overexpressed in tissue remodeling sites associated with inflammation, fibrosis, rheumatological diseases, atherosclerosis, and cardiac injury, among others 10,11 . 68 Ga-FAPI PET has shown good application prospects in imaging benign bone, joint, or muscle diseases, including immunoglobulin G4–related disease, 12,13 polymyositis, 14 rheumatoid arthritis, 15 osteoarthritis, 11,16,17 bone cyst, 18 fracture, 19 myositis ossificans, 20 Schmorl node, 21 osteofibrous dysplasia, 16,22 intraosseous meningioma, 23 and bone tuberculous 24 . The histopathological features of SAPHO syndrome are nonspecific osteomyelitis with inflammatory cell infiltration and a healing process with osteosclerosis and bone marrow fibrosis 4,25,26 .…”
