IntroductionMicroRNAs are small noncoding genes with gene expression regulatory function. Their emergence as potential diagnostic biomarker for many diseases has gained a specific interest among researchers. Observations of changes in miRNA levels correlating with aneuploidy in early embryos raise the prospective of employing miRNA as biomarkers to assess the embryo quality.MethodTo identify and gather the miRNAs with potential link to chromosomal abnormalities in embryos from previous research, we conducted a systematic search using four databases, including Embase, Medline, Web of Science, and Cochrane databases in accordance with PRISMA guidelines.ResultsOut of 200 identified records, only seven met the inclusion criteria. Seven miRNAs: miR-19b, miR-517c, miR-518e, miR-522, miR-92a, and miR-106a exhibited persistent downregulation in aneuploid blastocysts in the included studies. These miRNAs are members of important miRNA clusters, associated with abnormal expression in studies on reproductive failure. Pathway analysis revealed their involvement in regulating gene transcription, as well as cell cycle progression and apoptosis.DiscussionThe changes detected in the miRNA expression in aneuploid embryos across different studies support the aneuploidy and miRNA relationship and prospect miRNA as a valuable tool for the assessment of embryo quality. Collectively, these observations highlight the role of miRNAs in embryonic development, and their involvement in genetic abnormalities that occur in embryos, such as aneuploidy, indicating their potential implementation to improve the embryo selection and reproductive outcomes.

Неудачи и дефекты имплантации после ЭКО во многом определяются материнскими факторами, которые считаются достаточно управляемыми. Общая эффективность ЭКО, благодаря накоплению коллективного опыта и внедрению результатов передовых исследований в практику, неуклонно растет, но говорить о полной победе над инфертильностью пока слишком рано. До настоящего времени четко не определена врачебная тактика при выявлении наиболее значимых из них у пациенток, имеющих неудачи и дефекты имплантации в предыдущих программах ЭКО. Цель исследования. Обобщить данные литературы и результаты многолетних собственных исследований для определения врачебной тактики у пациенток с неудачами и дефектами имплантации в предыдущих программах ЭКО при выявлении значимых материнских факторов риска Материал и методы. Проведен анализ и обобщение данных 80 источников литературы с акцентом на работы отечественных авторов, включая публикации по результатам собственных исследований. Результаты. Выявлены наиболее значимые материнские факторы риска неудач и дефектов имплантации после ЭКО: хронический эндометрит и тромбофилии. Определен алгоритм прегравидарной подготовки у пациенток с хроническим эндометритом и с предыдущими неудачами и дефектами имплантации после ЭКО, включающий применение антибактериальной, противо вирусной, противовоспалительной, иммуномоделирующей и физиотерапии. Показана необходимость назначения низкомолекулярных гепаринов у пациенток с тромбофилией с момента установления беременности после ЭКО. Заключение. Проведение прегравидарной (предпрограммной) подготовки у пациенток с хроническим эндометритом и назначение низкомолекулярных гепаринов с ранних сроков беременности у пациенток с тромбофилией позволяет увеличить процент женщин, получивших беременность после ЭКО и преодолевших эмбрионический порог.

Cumulus cells are essential during oocytes growth and development, as well as during their maturation and fertilization. Research results have shown that embryo co-cultivation with autologous cumulus cells increases the frequency of blastocyst formation, and also improves the effectiveness of ART programs. Embryo transfer in such programs is recommended to be carried out using the CAT technology (Cumulus-Aided embryo Transfer), which includes embryo cultivation on a layer of cumulus cells and embryo transfer with a certain amount of diluted cumulus cells. Patient G., 38 years old, came to the department with infertility for 15 years and recurrent implantation failure in history. The patient had ART program with autologous co-cultivation of embryos with cumulus cells and a new CAT transfer technology. The patient fell pregnant and gave birth to a healthy child. Autologous cumulus cells can be a source of biologically active substances and improve embryological parameters and implantation rate in ART programs. Embryo co-cultivation with cumulus cells is especially important for patients with recurrent implantation failure. This technique can become an alternative for optimizing human embryos culturing.
