Empty pelvis syndrome: a systematic review of reconstruction techniques and their associated complications

Abstract: Pelvic exenteration (PE) is a radical procedure involving the resection of multiple pelvic viscera for primary and recurrent pelvic malignancies. The median 5-year overall survival for those with locally advanced and locally recurrent rectal cancers undergoing PE has been reported at 38% and 28%, respectively [1,2], and for gynaecological cancers it is 40% [3]. In the case of colorectal cancer, this compares with a 5-year survival of <5% without resection [4].

“…Admittedly, there is an absence of strong evidence proposing the superiority of one strategy over another in addressing the empty pelvis syndrome, with variations in practice across centres [10, 14]. However, in comparison to other techniques, BioA mesh is a relatively simple and straightforward strategy that requires little additional management considerations.…”

“…Reconstructive methods include myocutaneous flap reconstruction, omental flaps and mesh reconstruction. There is currently insufficient evidence in the literature to support the use of one reconstructive method over another [ 100 ].…”
