Endobronchial Capillary Hemangioma: Case Report and Review of the Literature

Abstract: A 66-year-old man with mitral stenosis on coumadin presents with hemoptysis caused by a capillary hemangioma of the proximal airways. Argon plasma coagulation was utilized to treat the lesions resulting in resolution of hemoptysis. Tracheobronchial capillary hemangiomas are rare in adults, but are easily discovered and treated with bronchoscopic intervention. The literature to date is reviewed pertaining to adult tracheobronchial capillary hemangiomas.

“…Carcinoid is accompanied with calcification in up to 30% of cases [15], and this may be useful for distinguishing carcinoid from glomus tumor. Hemangioma may also show marked enhancement effect [16]. Although it is indubitably difficult to distinguish these markedly enhanced tumors on dynamic contrastenhanced CT, the CT image provides useful information that the tumor is well vascularized and attention should be paid to biopsy.…”

“…Although several other reports have described resolution of hemoptysis following APC treatment of endoluminal targets, bleeding episodes in most cases were mild to moderate, with benign underlying causes, namely endobronchial hemangiomas and endobronchial endometriosis with catamenial hemoptysis [101,102,103]. …”

“…Because of the flattened shape and the limited size these lesions did not cause any significant obstruction. Overall eight cases have been reported in the literature with different morphology [6]: only in one previous case a pedunculated hemangioma obstructing the left lower lobe was reported [7]. The present case is characterised by a late onset presentation with non-specific symptoms masked by the underlying smoke-related chronic airway obstruction.…”

“…The present case is characterised by a late onset presentation with non-specific symptoms masked by the underlying smoke-related chronic airway obstruction. The unusual polipoid appearance added complexity to the problem of differential diagnosis [2,3,6]. Surgical excision of such tumors is associated with a favourable prognosis [7].…”
