Endocervical Curettage at the Time of Colposcopic Assessment of the Uterine Cervix

Abstract: After completion of this article, the reader should be able to list the proposed indications for endocervical curettage (ECC), to summarize the data in support of the routine use of ECC at the time of colposcopy, and to outline the evidence against the use of routine ECC at colposcopy.

“…Endocervical sampling is a collection of cells from the endocervical canal using either an endocervical curette (endocervical curettage; Figure 3) or a cytobrush 24 . Endocervical sampling has the potential to detect high‐grade lesions or invasive cancer that are not detected by colposcopy alone 26 . Endocervical sampling is less commonly used, because excisional procedures such as loop electrosurgical excision procedure (LEEP) or coninization have largely replaced ablative treatment (cryotherapy or laser ablation), and invasive cancer would likely be diagnosed upon pathologic examination of the tissue removed during excision 26 .…”

“…12,13 Unsatisfactory colposcopic examination as unseen squamocolumnar junction was supplemented by endocervical curettage histopathological examination was done for collected biopsy. 14 Collected data was statistically analysed to determine and to compare the sensitivity, specificity, positive predictive value and negative predictive value of VIA and Pap smear in screening of premalignant cervical intraepithelial lesion and subclinical early cancer cervix.…”

“…This latter procedure, the endocervical curettage (ECC), involves circumferentially scraping the endocervical canal. Although several reviews have summarized the findings of numerous heterogeneous studies [16], the use of ECC remains controversial [7]. …”
