Objective The high prevalence of chronic kidney disease in the community, especially in older patients, makes their management an important aspect of clinical practice. The aim of this study was to evaluate the knowledge attitudes and performance of Iranian endodontists regarding patients with chronic kidney disease (CKD). Methods This cross-sectional, descriptive-analytical study involved 100 Iranian endodontists (41 M, 59 F), who completed questionnaires with 21 items on CKD. Responses were analyzed by SPSS, using Chi-square or Fisher's exact tests. Results Overall levels of knowledge were rated as optimal in 55% of subjects, while attitudes to care were favourable in 83%. This cohort of specialist clinicians had strong scores for optimal attitude and performance. Awareness of issues around prescribed analgesics, antibiotic cover, bleeding risk and hypertension was high. Proximity to specialist training (younger specialists, and less years in practice) and female gender were associated with significantly higher scores across the three domains measured. Conclusion Most specialists were aware of requirements for safe care of dental patients with CKD. Continuing professional education should target older clinicians who may have had less exposure to formal training in this topic during their clinical training.

Symptomatic irreversible pulpitis and apical periodontitis in mature permanent teeth present challenges in their management. Traditional treatment approaches, such as root canal therapy or tooth extraction, may compromise tooth structure and oral function. This review article explores the combination of non-surgical endodontic treatment and vital pulp therapy as an alternative approach for these conditions. The purpose is to examine this combined approach's effectiveness, benefits, challenges, and limitations. The objectives include reviewing the literature, evaluating clinical outcomes, discussing potential benefits, and providing recommendations for clinical practice. The combination approach aims to preserve tooth structure, promote healing, and reduce postoperative complications. The article discusses the rationale for combining the two techniques, presents evidence supporting their efficacy, and outlines the techniques and protocols involved. Clinical outcomes, case studies, potential challenges, and comparative analysis with traditional approaches are also explored. Future directions and research recommendations highlight areas for further investigation, innovations, and the development of clinical guidelines. In conclusion, the combination of non-surgical endodontic treatment and vital pulp therapy offers a valuable strategy for managing mature permanent mandibular molars with symptomatic irreversible pulpitis and apical periodontitis. Further research and advancements are needed to refine the treatment protocol and expand the evidence base, and clinicians should stay updated to provide optimal care and improve patient outcomes.

Background: Wolff-Parkinson-White syndrome is a congenital abnormality of the heart. Patients with this syndrome and multiple dental caries with pulp exposure may require comprehensive dental treatment under general anesthesia. Pulpectomy is performed in primary teeth with pulp expose. This treatment in anterior teeth requires palatal access and the majority there is no good visibility, requiring more time to perform. Therefore, this case aimed to demonstrate a pulp chamber opening alternative in pulpectomies in a child with Wolff-Parkinson-White syndrome.Case Presentation: A 3-year-old female patient with Wolff-Parkinson-White syndrome and multiple dental caries with reversible pulpitis in anterior upper teeth. The dental treatment was performed under general anesthesia. Due to abundant hemorrhage in anterior upper teeth, pulpectomies were performed, which pulp chamber openings were modified by labial and the treatment itself was completed. After, these teeth were rehabilitated with direct restorative veneers using light-curing composite resin. The follow-up at 12 months showed an absence of evidence clinic and radiographic. Conclusion: This case demonstrated the pulp chamber opening performed by labial in pulpectomy is an effective and viable alternative to reduce operative time and improve visibility in pulpotomies in a child with Wolff-Parkinson-White syndrome.
