Endometrial Thickness- a Practical Prospective Marker for the Risk of Surgical Intervention after RU486 Induced Abortion

Abstract: Background: Medical termination of pregnancy [TOP] during the early fi rst trimester is commonly used. However, treatment failure which warrants surgical intervention occurs in small proportion of patients. Our objective was to examine the effectiveness and predictive value of sonographic measurement of endometrial thickness during a follow up visit after medical abortion as an accurate predictor of the necessity of curettage for completion of pregnancy termination.

“…Receptive endometrium is an important contributor for the success of pregnancy, and it is adequately primed to implant the embryo. Receptive endometrium growth is under the control of steroid hormones, growth factors and cytokines in addition to rich blood supply, which are required for an optimal implantation …”

“…Ultrasonography has an increasingly important role in the evaluation and treatment of infertile patients . Pulsed and colour Doppler ultrasonography are used to assess for different variables of uterine and endometrial/sub‐endometrial perfusion, which can ultimately affect the endometrial receptivity…”

“…There is no difference between 11 and 14 mm regarding the risk of surgical intervention after medical TOP. 21 An endometrial mass is the most sensitive finding for RPOCs, as per the study by Blumenfeld F et al 20 If no mass or endometrial fluid is seen and the endometrial thickness is less than 10 mm, RPOCs are extremely unlikely. However, in our study, in cases where ET was between 7 and 10mm, majority of patients' specimen (64.29%) showed evidence of products of conception.…”
