Endoscopic Cycloplasty (ECPL) and Lens Extraction in the Treatment of Severe Plateau Iris Syndrome

Abstract: Treating the ciliary processes with diode laser using an endoscopic approach (ECPL) and lens extraction, results in opening of the anterior chamber angle and shrinkage of the ciliary processes in the treated areas. This effect seems to be primarily a result of the laser treatment in combination rather than the lens extraction alone. This effectively reverses the anatomic cause and angle closure of plateau iris syndrome.

“…Given the mechanical changes in ciliary process height, the authors of this review have utilized ECP to produce an endocycloplasty effect in patients with plateau iris syndrome, which was first described in 2009 [24]. Francis et al [25] describe a small series of patients in whom this technique was utilized, with resultant angle opening as evidenced by objective ultrasound biomicroscopic metrics.…”

“…This achieves a 61% IOP reduction but is currently reserved for refractory glaucoma due to increased risk for inflammation. 49 More clinical data are needed, but it is likely that patients with other risk factors for CME, such as diabetes or uveitis, are not ideal candidates for ECP.…”

“…Endo cycloplasty is a technique where shrinkage of ciliary processes with photocoagulation also helps in widening the AC angle in PIS and for treatment of irido ciliary cysts causing angle closure. [ 143 144 ] This is generally combined with a lens extraction.…”
