Endoscopic Mucosectomy in a Child Presenting with Gastric Heterotopia of the Rectum

Abstract: Gastric mucosal heterotopia has been described in all levels of the gastrointestinal tract. Its occurrence in the rectum is uncommon. We report the case of a 4-year-old boy referred to Pediatric Gastroenterology for intermittent rectal bleeding for the past 2 years. Total ileocolonoscopy revealed a flat, well-circumscribed lesion of 4 cm, with elevated margins, localized at 10 cm from the anal verge. Histologic examination showed typical gastric mucosa of the oxyntic type. Treatment with proton pump inhibitors… Show more

“…8 At least 4 pediatric cases have also been treated with endoscopic mucosal resection without complications. 10 There is inconclusive evidence about whether HGM can progress to malignancy, but the potential is present. There are only 3 adult cases in the literature reporting rectal HGM with metaplasia.…”

“…Many of these polyps, however, are pedunculated and do not require an EMR technique for resection. The published literature includes case reports of EMR for gastric, small intestine and colonic lesions with technical success and no adverse events (16)(17)(18). The largest published pediatric series to date compares therapy with high-frequency electrocoagulation to EMR in a series of 154 children in Guiyang Children's Hospital in China.…”

“…Endoscopic resection or surgical excision is generally considered the treatment of choice and has resulted in complete relief following removal of the HGM. Conventional endoscopic interventions such as endoscopic snare resection (polypectomy or endoscopic mucosal resection[ 20 ]) and ablation with APC are indicated in HGM patients mimicking polyps and small superficial lesions. ESD, enables en bloc resection of superficial lesions especially in early GI cancer and LST, and could also be used in the treatment of HGM.…”
