Osseous metaplasia (OM) in the stomach is a rare finding, with less than 10 cases reported in the English literature to date. We present a rare case of a gastric adenoma with high-grade dysplasia and stromal OM in a 65-year-old man. The clinical, endoscopic, and histologic features along with a review of the literature are discussed. To the best of our knowledge, this is the first reported case of identifiable OM in a gastric polyp radiologically.
