Endoscopic transcanal middle ear surgery

Abstract: The advantages and limitations of the microscope of choice for the treatment of diseases of the middle ear. The wide-angle view provided by the endoscope enables transcanal access to the tympanic cavity, and its otherwise recess, and hypotympanum. These areas are the primary sites of disease and surgical failure to cure. The endoscope also allows an all encompassing view of the three main elements in tympanoplasty surgery: The ear canal, tympanic membrane, and the tympanic ring. This report is a summary transc… Show more

“…Thus, the deep anterior canal wall, anterior recess, anterior marginal perforations, sinus tympani, facial recess, hypotympanum and attic are visualized by rotating the angled scopes. [3][4][5][6] Very few studies have been conducted till date to correlate the outcomes of microscopic assisted and endoscopic assisted tympanoplasty. Hence this study is to compare the outcomes of tympanoplasty by conventional microscopic assisted method and endoscopic assisted method.…”

“…In an original article, "Endoscopic transcanal middle ear surgery" by Tarabichi M,92 % success in endoscopic transcanal success rate in graft closure was seen compared to microscopic surgery which was 80%. The advantage of endoscope was, it could be easily negotiated behind the narrow segment of external auditory canal and provides a panaromic, wide angled, magnifi ed view that allows the surgeon to "look around corners" unlike microscope 5 . One handed technique and loss of depth perception are the disadvantages of endoscopic technique 6 .…”

“…However, endoscopes have been used for observation and treatment of conditions in the areas that may be hard to visualize completely under the microscope. [6][7][8] In treatment of cholesteatoma, we have employed an endoscope combined with a microscope to minimize drilling, thereby preserving hearing and facial nerve called as "endoscopy assisted surgery". 10 Few reports have been published concerning stapes surgery that is entirely performed by endoscopy.…”
