Endoscopy for the initial suspicion of vascular rings in tracheoesophageal compressions: Correlation with surgical findings

Abstract: Initial suspicion of VR was raised by bronchoscopic and/or esophagoscopic findings in 60% cases. Endoscopy proved to be a valuable tool to anticipate the type of malformation and to disclose associated abnormalities.

“…The initial diagnosis of innominate artery compression is nearly always made with bronchoscopy, classically showing a pulsatile anterior compression of the trachea extending from left to right at a location closer to the vocal cords; in particular, it has been defined significant when the extent of the compression is more than 70–80% of the lumen of the trachea . In cases of pulmonary artery sling, bronchoscopy is required to assess for associated complete tracheal rings …”

“…In anatomyassociated anomalies, different pathologic lesions are associated with unique bronchoscopic and radiographic findings. 15 Involvement of the upper trachea is always found in children with a double aortic arch. In pulmonary arterial sling lesions, the course of the left pulmonary artery usually results in compression of the lower trachea until the carina and right main bronchus, sometimes simultaneously with the left main bronchus.…”

“…Although endoscopy has been proposed helpful in the diagnosis of the condition [2,11,12], there is only a single report of its implication in the treatment of dysphagia caused by ARSA [13]. Given its more widespread availability, ease of performance, lower complication and morbidity profile, and lower cost compared to surgical repair, endoscopic dilation may be considered a suitable treatment alternative in patients who are not a surgical candidate or do not consent for surgery.…”
