Enforcing the 4T: Including a Hard Stop 4T Calculator into the Electronic Medical Record for Heparin Induced Thrombocytopenia

Abstract: Background: Among many causes of thrombocytopenia acquired during hospitalization, heparin-induced thrombocytopenia (HIT), an immune complex-mediated thrombogenic state with high morbidity and mortality, is one of the most feared etiologies. The clinical '4T' score is an established method to assess risk for HIT and guide immediate management(Lo, Juhl et al. 2006). However, the 4T score is not consistently used by clinicians, leading to potential over-testing and suboptimal management of suspected HIT. Our obj… Show more

“…Implementation of a 4Ts score calculator in the electronic medical record (EMR) ordering system may reduce testing via mandatory 4Ts score calculation prior to each HIT test. [5][6][7] The primary outcome of this quality improvement project is differences in HIT testing rate pre-and post-implementation. Secondary outcomes included changes in the proportion of positive vs negative HIT tests and the level of "inappropriate" testing (ie, low pre-test probability) post-implementation.…”
