AIM:This study explored the effect of 786TC (rs2070744) genetic polymorphism of the NOS3gene on main indices of systemic hemodynamics and heart rate variability at active orthostatic test in Northern men. METHODS:Eighty-three practically healthy men, all permanent residents of Magadan Region. The average age was 33.51.5 yrs. The examinees went through the resting and active orthostatic testing modes which provided recording cardiovascular system variables using an automatic tonometer with further calculation of hemodynamic indices, and simultaneous registering short-term heart rate variability in frequency and spectral areas using the Varikard hardsoft complex unit. RESULTS:For analysis, the surveyed cohort was divided into two groups : Group 1 was for TT (n=34) and Group 2 carriers of the NOS3*Сallele variant with genetically reduced nitric oxide production (n=49). Analysis of the complex of vasomotor, chrono- and inotropic reactions of hemodynamic and heart rate variables under the active orthostatic test allowed for identifying different patterns of the two examined groups response. Men from Group 1demonstrated an increased cardiac output contribution to the orthostatic reaction, with vascular resistance values remaining constant. That was observed with a pronounced sympathetic autonomic shift and growth in the very low-frequency component of general heart rate variability, which apparently happened owing to higher NO activity in the cardiovascular system. Group 2, with sympathetic insufficiency accompanied by raised vascular resistance in response to the orthostatic test, exhibited the 35% growth in the low-frequency component of the cardiac rhythm, which confirmed the role of that indicator as an activator of blood pressure rhythm fluctuations realized through baroreflective mechanisms. CONCLUSION: The conducted studies showed that, men whose genotype did not have the NOS3*С allele variant demonstrated more favorable responses to the active orthostatic test, which may indicate a sufficiently high level of their cardiovascular functional reserves. The 786TC (rs2070744) polymorphism of the NOS3 gene proved to be an informative marker for assessing subjective cardiohemodynamic condition and autonomic readjustments under functional exercise, particularly an active orthostatic test.

Проведено молекулярно-генетическое обследование 101 мужчины, рожденных или проживающих на территории Магаданской области, преимущественно европейцев. В генофонде популяции представлено 2 аллеля по локусу NOS3: NOS3*Т, частота которых составила 64,36% и 35,64%, соответственно. В популяции равновесие Харди - Вайнберга не нарушено (χ2(HWE) = 0,26, p>0,05). Проанализировано влияние полиморфизма -786 Т>С (rs 2070774) гена NOS3 на степень напряжения в работе сердечно - сосудистой системы на основании данных об основных показателях системы гемодинамики. В целом, полученные нами результаты свидетельствуют о том, что аллель NOS3*С приводит к напряжению в работе сердечно - сосудистой системы, что может быть использовано для расчета риска формирования сердечно-сосудистой патологии уже в молодом возрасте в экстремальных условиях Севера. Ключевые слова: генетический полиморфизм, гемодинамические показатели, жители-северяне.
