Epidural anaesthesia for Caesarean section in a patient with von Hippel-Lindau disease

Abstract: Von Hippel-Lindau disease is a rare autosomal dominant disease with incomplete penetrance and variable expression and is characterized by diffuse haemangioblastomas of the central nervous system and viscera. The majority of the central nervous system lesions are located in the cerebellum. This report describes the successful management with epidural anaesthesia of a woman with a term gestation, von Hippel-Lindau disease and cerebellar haemangioblastoma.

“…Although regional anesthesia is usually employed for cesarean sections, a standard anesthetic method for systemic VMs, such as BRBNS and Von Hippel-Lindau disease, remains controversial, particularly in cases of nervous system involvement [1315]. General anesthesia was selected because of the following reasons: MRI revealed no VMs in the patient's air way, direct visual guidance during the procedure is preferred, and spinal or epidural anesthesia may lead to secondary spinal epidural hematoma induced by direct or indirect injury to the vasculature [16].…”

“…3,4 VHL is inherited in an autosomal dominant fashion and arises secondary to a mutation in the VHL tumour suppressor gene involved in cell cycle regulation and angiogenesis. 5 The incidence is one in 35,000-65,000. 6 Haemangioblastomas may occur at various sites within the central nervous system although the cerebellum is the commonest location.…”

“…9 There is a reported 3-10% recurrence rate after resection and long-term follow-up for these patients is therefore advised. 5 The progress of asymptomatic haemangioblastomas in pregnancy is more difficult to determine from the literature. Grimbert et al looked retrospectively at 56 pregnancies in 30 women with VHL of whom four had central nervous system haemangioblastomas.…”
