Epithelial antigenic specificities of circulating autoantibodies in mucosal lichen planus

Abstract: Some patients with mucosal LP may present with circulating anti-BP180 antibodies at low levels. We also report the first case with positive anti-Dsg3 antibodies. The pathogenic relevance of these autoantibodies remains unknown.

“…The repetitive and continuous damages caused by the chronic and erosive dermatitis in our patient's genital skin might have broken the organ-specific immune tolerance and sensitized it to the autoantigens in the epidermis. Consistently, the production of autoantibodies against BP180, Dsg3 and Dsg1 has been reported in lichen planus, [1][2][3] in which the basement membrane zone and keratinocytes are certainly damaged. A similar relationship has been proposed in pemphigoid nodularis preceded by prurigo nodularis.…”

“…[1][2][3] Here, we report a case with refractory erosion on the genital skin that preceded the appearance of autoantibodies against desmoglein (Dsg)1 and the development of pemphigus foliaceus (PF). “…Several hemidesmosomal proteins may serve as autoantigens. Collagen α ‐1(XVII) chain‐reactive T cells have been found in dermal lichen planus, and autoantibodies towards collagen α ‐1(XVII) chain antibodies have been described in oral lichen planus (51–54). Autoantibodies have been found to be reactive with integrin‐ α 6 in oral pemphigoid, as well as with the collagen α ‐1(XVII) chain and dystonin in mucous membrane pemphigoid (55), bullous pemphigoid and in elderly patients with chronic non‐bullous pruritic disorders (56).…”

“…Furthermore, within the PV buccal mucosa biopsy there was a distinct area of hyperkeratosis with an associated band-like infiltrate resembling a lichenoid tissue reaction, however the significance of this is unknown. Interestingly, within the literature there are cases of oral lichen planus with circulating anti-Dsg3 antibodies, which are typically characteristic of PV [ 35 , 36 ]. The pathogenic role of these antibodies in lichen planus however, is not yet understood.…”
