Erectile dysfunction and heart failure: the role of phosphodiesterase type 5 inhibitors

Abstract: The phosphodiesterase type 5 (PDE-5) inhibitors are effective in treating erectile dysfunction (ED). ED and heart failure (HF) share similar risk factors, and commonly present together. This association has led to questions ranging from the safety and efficacy of PDE-5 inhibitors in HF patients to a possible role for this class of medication to treat HF patients with or without ED. In addition to endothelial dysfunction, there are causes of ED specific to patients with HF including low exercise tolerance, depr… Show more

“…Unfortunately, PDE-5 inhibitors show negative interactions with some of the drugs used in the treatment of these diseases (e.g., nitroglycerine, doxazosin and terazosin) [3,4]. Lots of evidences are coming into literature about adulteration of HDSs with synthetic drugs or their congeners, in order to enhance the claims stated on the label.…”

“…This protection persisted for 36 h, and a subsequent readministration of tadalafil at 36 and 72 h restored the protection until 108 h, with a possible role of protein kinase B (Akt) signaling (2). PDE inhibitors have been extensively used for treatment of heart failure, pulmonary hypertension, and coronary artery disease besides their use for the treatment of erectile dysfunction (3,12,16,23). Inhibition of PDE5 in the hypertrophied myocardium significantly improved myocardial contractility (17).…”

“…In clinical trials PDE5 inhibitors seem to benefit patients with congestive heart failure, especially those with secondary pulmonary hypertension. 92 In contrast to on-demand dosing, chronic daily dosing of PDE5 inhibitors provides marked, sustained improvement in peripheral endothelial function and has improved ED in some nonresponders to on-demand dosing. 93 A variety of other medications, supplements and lifestyle habits have been shown to have a beneficial effect on endothelial function ( Figure 6).…”
