ESHRE good practice recommendations on recurrent implantation failure

Abstract: STUDY QUESTION How should recurrent implantation failure (RIF) in patients undergoing ART be defined and managed? SUMMARY ANSWER This is the first ESHRE good practice recommendations paper providing a definition for RIF together with recommendations on how to investigate causes and contributing factors, and how to improve the chances of a pregnancy. WHAT IS KNOWN ALREADY … Show more

“…Of various potential causes of implantation failure, except for embryonic factor, evidence-based treatments for RIF include normalization of intrauterine abnormalities including CE, thyroid dysfunction, and thrombophilia; lifestyle modification; and preimplantation genetic testing for chromosomal abnormalities. 31 The standard treatment options for RPL include only levothyroxine supplementation for hypothyroidism and low-dose aspirin and low-molecular-weight heparin for antiphospholipid syndrome. 16 However, treatment for unexplained RPL has not been established.…”

“…Many strategies such as estrogen, low-dose aspirin, heparin, vaginal sildenafil, pentoxifylline, and granulocyte-colony stimulating factor (G-CSF) intrauterine perfusion have been extensively used to increase the ET if not optimal.6,7 However, these methods were not found to be very impressive in all cases especially with a thin refractory endometrium. Platelet-rich plasma (PRP) may be effective in promoting endometrial growth, increasing ET and improving endometrial vascularity, and improving pregnancy outcomes in repeated implantation failure due to thin endometrium [7].…”

“… 1 , 2 In recent years, evidence for each examination and treatment approach has been reviewed, resulting in the publication of guidelines for recurrent implantation failure (RIF). 3 Despite the lack of a consensus on test selection and treatment approaches, combination treatment based on diagnostic results from selected examinations is required for RIF, which is considered a multifactorial disease. To achieve pregnancy during the next embryo transfer (ET), this review seeks to clarify evidence‐based testing and treatment strategies after implantation failure following euploid ET.…”
