Establishing benchmarks for the outcome of herniotomy in children

Abstract: Complication rates were relatively low compared with those reported previously, but were considered achievable in most paediatric surgical units. Specific attention should be paid to the youngest infants, as this group has the highest complication rate. Minimum standards can be proposed based on these and other published outcomes.

“…However, a longer period of follow-up would be needed to assess for other outcome measures in order to compare with results of other researchers. [913] The fact that there was no significant increase in the duration of the surgery with closure of the spermatic fascia indicates that it is not more technically demanding to close the spermatic fascia. A basic principle in surgery is to put structures back to the original anatomic state whenever possible.…”

“…Inguinal hernia repair and other inguinal region operations are the most common operations performed by pediatric surgeons [1]. Iatrogenic vas deferens injuries (IVDI) are one of the most serious complications of these operations [2]. IVDI may develop as a result of direct transection or excessive compression of the vas deferens (VD).…”

“…Regardless of the mechanism, oligospermia or azoospermia may develop and result in infertility in these patients in later years [3]. The incidence of IVDI during inguinal hernia repair in children has been reported to be 0.05%-1.6% [2,4]. The frequency of VD obstruction in subfertile patients with a history of inguinal hernia repair in childhood reaches 26.7% [5].…”

“…6 Consequently, COIH has continued to dominate pediatric surgeons' practice because of its long track record of effectiveness, safety, and simplicity. 7,8 To achieve the advantages of both procedures, we are able to modify the COIH technique on selected patients to perform safe inguinal herniotomy through smaller incisions. This report provides detailed description of the mini-scar inguinal herniotomy (MSIH) technique in addition to a preliminary comparative assessment of the clinical outcomes and parental attitudes in comparison with a matched group of COIH.…”
