Estrogen-based hormone therapy in women with primary ovarian insufficiency: a systematic review

Abstract: Purpose Sex hormones play a role in bone density, cardiovascular health and wellbeing throughout reproductive lifespan. Women with primary ovarian insufficiency (POI) have lower estrogen levels requiring hormone therapy (HT) to manage symptoms and to protect against adverse long-term health outcomes. Yet, the effectiveness of HT in preventing adverse outcomes has not been systematically assessed. We summarize the evidence regarding effects of HT on bone and cardiovascular health in women with POI. Methods A … Show more

“…Sex steroid hormone replacement therapy is indicated in all girls and women age < 40 with hypogonadism, either due to primary gonadal failure or due to hypothalamic-pituitary failure. Estrogen replacement will restore pubertal development and may remediate or prevent the consequences of premature estrogen deficiency such as loss of bone mass, loss of self-esteem and decrease in sexual activity [49]. There is insufficient evidence that androgen replacement therapy in women with POI has beneficial effect on sexual function, bone mineral density, or quality of life [50].…”

“…Estrogen, a steroid hormone in the human body, plays an important role in many physiological processes,[ 1 ] such as the maintenance of secondary sexual characteristics, the development of neurons, and bone homeostasis. [ 2 ] Estrogen is critical to maintain bone and bone density. [ 3 ] Estrogen deficiency often leads to the emergence of osteoporosis.…”

“…The adult patients with APECED were shorter and their BMI was lower than in controls subjects, which may explain partly the differences in bone architecture. Women with POI have increased risk of fractures and osteoporosis, but the current body of evidence of hormone replacement therapy on fracture risk and bone health is limited (23). In women with PAI who have not hypoparathyroidism, postmenopausal state increases the risk of bone loss, and glucocorticoid replacement therapy must be particularly carefully monitored (12).…”
