PURPOSE:To evaluate the maximal intraluminal pressure (MIP) supported by canine cadaveric urinary bladders that underwent cystotomy followed by cystorraphy, with and without serosal patching-supplementation. METHODS:Two groups (n=8 each) were formed, and in one (conventional) the cystotomy was closed with cushing pattern. In the other group (serosal), the same procedure was performed, and a piece of jejunum was used for the construction of the serosal patching over the cystorraphy. MIP was measured by means of an invasive blood pressure transducer with closed stopcock attached to a multiparameter monitor. At the end of each measurement, the bladder body circumference was assessed. RESULTS:Mean±SD MIP sustained for the conventional and serosal groups were 28.88±5.08 and 65.38±10.99 mmHg, respectively (p<0.0001). Bladder circumference did not change significantly between groups (p=0.35) and did not correlate with MIP assessed in conventional (p=0.27; r=0.4379) and serosal groups (p=0.37; r=-0.3637). CONCLUSION:Serosal patch-supplemented cystorraphies were able to sustain intraluminal pressures 55.8% higher, than nonsupplemented cystorraphies in specimens from canine cadavers.

OBJETIVO: Estudar aspectos da cicatrização na anastomose colônica com adesivo de fibrina em ratos normais e diabéticos. MÉTODO: Foram utilizados 160 ratos Wistar (Rattus novergicus albinus), machos, com peso variando de 250 a 300g, sendo que oitenta destes animais eram não diabéticos e os outros oitenta animais apresentavam diabetes mellitus induzido pela aloxana, Os animais foram submetidos a procedimento operatório padronizado que consistiu em colectomia parcial esquerda e anastomose primária por sutura manual ou com adesivo de fibrina. Foram reavaliados no quarto ou sétimo dia de pós-operatório quando se observou a presença de coleção líquida ou purulenta na tela subcutânea; integridade da sutura; presença de aderências; presença de deiscência; resistência da anastomose (pressão máxima de insuflação) em mmHg, e a concentração tecidual de hidroxiprolina. RESULTADOS: Em relação à presença de deiscência de anastomose e mortalidade ocorreram piores resultados nos animais diabéticos no quarto e sétimo dias de observação em comparação ao controle. Nos grupos e subgrupos estudados os resultados com o uso do adesivo de fibrina não foram melhores que os da sutura manual. Em relação à concentração tecidual de hidroxiprolina não houve diferença significativa entre os grupos. CONCLUSÃO: O diabetes mellitus induzido pela aloxana impede a reparação normal da anastomose colônica em ratos e o uso adesivo de fibrina na anastomose colônica em ratos normais e diabéticos não trouxe benefícios na evolução pós-operatória.

ObjectiveTo evaluate the behavior of castor oil-derived polyurethane as a hemostatic agent and tissue response after abdominal aortic injury and to compare it with 2-octyl-cyanoacrylate.MethodsTwenty-four Guinea Pigs were randomly divided into three groups of eight animals (I, II, and III). The infrarenal abdominal aorta was dissected, clamped proximally and distally to the vascular puncture site. In group I (control), hemostasis was achieved with digital pressure; in group II (polyurethane) castor oil-derived polyurethane was applied, and in group III (cyanoacrylate), 2-octyl-cyanoacrylate was used. Group II was subdivided into IIA and IIB according to the time of preparation of the hemostatic agent.ResultsMean blood loss in groups IIA, IIB and III was 0.002 grams (g), 0.008 g, and 0.170 g, with standard deviation of 0.005 g, 0.005 g, and 0.424 g, respectively (P=0.069). The drying time for cyanoacrylate averaged 81.5 seconds (s) (standard deviation: 51.5 seconds) and 126.1 s (standard deviation: 23.0 s) for polyurethane B (P=0.046). However, there was a trend (P=0.069) for cyanoacrylate to dry more slowly than polyurethane A (mean: 40.5 s; SD: 8.6 s). Furthermore, polyurethane A had a shorter drying time than polyurethane B (P=0.003), mean IIA of 40.5 s (standard deviation: 8.6 s). In group III, 100% of the animals had mild/severe fibrosis, while in group II only 12.5% showed this degree of fibrosis (P=0.001).ConclusionPolyurethane derived from castor oil showed similar hemostatic behavior to octyl-2-cyanoacrylate. There was less perivascular tissue response with polyurethane when compared with cyanoacrylate.
