Ethmoidal sarcoidosis

Abstract: Sarcoidosis is a multisystem granulomatous disease of unknown aetiology. Nasal sarcoidosis commonly affects the mucosa of the septum and the inferior turbinates. These patients may present with nasal discharge, crusting, obstruction, epistaxis or anosmia. We present an unusual case of nasal sarcoidosis involving the ethmoid sinus causing recurrent eyelid swelling and discuss its management.

“…Stage I includes bilateral hilar lymphadenopathy. In stage II, there is bilateral hilar adenopathy and pulmonary infiltration, and in stage III, the pulmonary infiltration is observed but there is no lymphadenopathy8. Thus, our patient was diagnosed with stage II sarcoidosis, because both adenopathy and pulmonary infiltration were observed.…”

“…2 It is rare for sarcoidosis to affect the sinonasal tract exclusively. 3 Most patients have multiorgan involvement at presentation because symptoms related to the nose or paranasal sinuses are minimal. 4 In some cases the initial manifestation of sarcoidosis may be detected in the sinonasal tract.…”

“…On the other hand, it is rare for sarcoidosis to affect the nose exclusively. Nasal sarcoidosis commonly affects the mucosa of the inferior turbinates and the septum and, sometimes [5], that of the paranasal sinuses, although they are rarely involved without any macroscopic changes in the nasal mucosa [4]. Maybe, paranasal sarcoidosis may be underestimated in patients with systemic sarcoidosis for an inaccurate management [7].…”

“…Although it may be disfiguring, cutaneous sarcoidosis rarely causes major problems. Topical preparations of corticosteroids have been reported to be successful in many cases, if the nasal symptoms are important [4]. Frequent nasal irrigation is recommended for those patients with nasal crusting [2].…”
