Ethyl Pyruvate as a Potential Defense Intervention against Cytokine Storm in COVID-19?

Abstract: COVID-19 is a deadly pandemic and has resulted in a huge loss of money and life in the past few months. It is well known that the SARS-CoV-2 gene mutates relatively slowly as compared to other viruses but still may create hurdles in developing vaccines. Therefore, there is a need to develop alternative routes for its management and treatment of COVID-19. Based on the severity of viral infection in COVID-19 patients, critically ill patients (∼5%, with old age, and comorbidities) are at high risk of morbidities.… Show more

“…This uncontrolled systemic inflammatory response causes immunopathological events, leading to severe pneumonia, ARDS, multiple organ failure, and ultimately the death of a patient with COVID-19 [36,37]. To avoid such a scenario, it is necessary to monitor the expression of proinflammatory mediators from the onset of infection and, accordingly, to search for alternative treatment strategies during the period when the human body still has enough time to regulate its immunity to fight the infection [38].…”

“…Thus, pyruvate-enriched fluids would be an optimal selection to prevent postoperative hyperglycemia, probably due to its preservation of PDH and islet function (30)(31)(32)(33)60). At present, if pyruvate solutions were compassionately used in severe COVID-19 patients, clinical outcomes might have greatly improved (86,87). In the future, pyruvate-enriched fluids may encompass other medical solutions specifically used for RBC storage, cell salvage, organ preservation, cardioplegia, peritoneal dialysis, priming fluid for cardiopulmonary bypass circuits, and others (24,27,77,80,88).…”

“… 76 It was newly further demonstrated that EP with glycyrrhizic acid through the inhibition of HMGB1 release and inflammatory senescence prevented Tau oligomers-induced cellular senescence and tauopathies, such as Alzheimer’s disease, in the brain of mice. 77 In this regard, EP was recently proposed in Covid-19 treatments, 78 as mentioned above due to susceptibility of cellular senescence to viral infection. 37 Similarly, pyruvate is also brain protective against tauopathy and sodium pyruvate was hypothesized in the intervention of Covid-19 viral infection, too.…”
