Aims: Perinatal mental illness (PMI) is associated with a high risk of maternal and infant morbidity. Recently, several systematic reviews and primary studies have explored the prevalence and risk factors of PMI in the Middle East and North Africa (MENA) region. To our knowledge, there has been no critical analysis of the existing systematic reviews (SRs) on this topic in the MENA region. Our systematic overview primarily aimed to synthesize evidence from the published SRs on PMI in the MENA countries focusing on a) the prevalence of PMI and b) the risk factors associated with PMI. Methods: We conducted a systematic overview of the epidemiology of PMI in the Middle East and North Africa region by searching the PubMed, Embase, and PsycInfo databases for relevant publications between January 2008 and July 2019. In addition to searching the reference lists of the identified SRs for other relevant SRs and additional primary studies of relevance (those which primarily discussed the prevalence of PMI and/or risk and protective factors), between August and October 2019, we also searched Google Scholar for relevant studies. Results: After applying our inclusion and exclusion criteria, 15 systematic reviews (SRs) and 79 primary studies were included in our overview. Studies utilizing validated diagnostic tools report a PMI prevalence range from 5.6% in Morocco to 28% in Pakistan. On the other hand, studies utilizing screening tools to detect PMI report a prevalence range of 9.2% in Sudan to 85.6% in the United Arab Emirates. Wide variations were observed in studies reporting PMI risk factors. We regrouped the risk factors applying an evidence-based categorization scheme. Our study indicates that risk factors in the relational, psychological, and sociodemographic categories are the most studied in the region. Conversely, lifestyle-related risk factors were less studied. Conclusions: Our systematic overview identifies perinatal mental illness as an important public health issue in the region. Standardizing approaches for estimating, preventing, screening, and treating perinatal mental illness would be a step in the right direction for the region.

Background: Postpartum mood disturbance affects up to 85% of women, with most symptoms of this disturbance being temporary and mild. However up to one in seven women experience a persistent type of depression which is associated with major maternal and neonatal morbidity if it is not managed. Objective: To estimate the prevalence of postpartum depression and to identify the associated risk factors. Methods: A cross sectional study was conducted in randomly selected 3 primary health care centers affiliated to Suez governorate, Egypt. An interviewed questionnaire was used containing potential risk factors for postpartum depression. An Arabic version of the Edinburgh postnatal depression scale (EPDS) was used to screen for postpartum depression symptoms. Results: A total of 237 postpartum mothers were included in the study. 139 (58.6%) of the mothers completed secondary level of education, 195 (82.3%) of them were housewives and 181 (76.4%) had cesarean section. The estimated postpartum depression prevalence was 26.6% and suicidal ideation accounted for 4.6%. Factors significantly associated with high EPDS scores were bad relationship with the husband, having >2 children, an unplanned pregnancy and unhealthy newborn; with P-values of 0.000, 0.004, 0.000, and 0.018, respectively. Discussion: the prevalence of postpartum depression among Egyptian women is considered slightly high with its negative and long term consequences on the mothers and their children, accordingly screening, proper management & referral to specialist care is highly recommended.

ResumoIntrodução: O período de tratamento e recuperação de uma criança hospitalizada pode ser longo. Durante esse tempo em que precisa estar inserida no ambiente hospitalar, muitas vezes institui-se uma crise na vida da criança e da mãe. Objetivo: Avaliar a prevalência de sintomas de ansiedade e depressão materna em mulheres que tiveram seus fi lhos internados na ala pediátrica do hospital universitário da Universidade Luterana do Brasil (ULBRA). Métodos: Este foi um estudo transversal, com aplicação dos Inventários de Ansiedade e Depressão de Beck (Beck Anxiety and Depression Inventories, respectivamente BAI e BDI) em 140 mulheres que se encontravam entre 31 e 365 dias após o parto. Adotamos o ponto de corte > 20 nas escalas BAI e BDI para defi nir a presença de ansiedade e depressão. Resultados: Do total, 43 mulheres (30,7%) apresentaram escores > 20 no BDI, ou seja, foram consideradas deprimidas; 46 (32,9%) apresentaram escores > 20 no BAI, tendo sido consideradas ansiosas; 26 mulheres (18,6%) apresentaram-se ansiosas e deprimidas. Mulheres em situação de maior vulnerabilidade social apresentaram depressão e ansiedade com maior frequência, especifi camente mulheres mais jovens, com menor escolaridade e menor renda, sem plano privado de saúde e com mais fi lhos. Conclusões: A atuação preventiva de equipes multidisciplinares, nos primeiros meses após o parto, é necessária devido à alta frequência de sintomas depressivos e de ansiedade nas mães. Com isso, essas mães que se encontram em um ambiente hospitalar podem ser encaminhadas para profi ssionais de saúde mental adequados. During the child's stay in the hospital environment, it is not uncommon to observe the development of emotional crises affecting both child and mother. Objective: To assess the prevalence of depression and anxiety symptoms among mothers who had their children admitted to the pediatric ward of a university hospital in southern Brazil. Methods: This was a cross-sectional study in which the Beck Anxiety and Depression Inventories (BAI and BDI, respectively) were administered to 140 women between 31 and 365 days postpartum. The cut-off point of > 20 on BAI and BDI was used to defi ne the presence of anxiety and depression. Results: Of the 140 women assessed, 43 (30.7%) presented BDI scores > 20, and were therefore considered to be depressed; 46 (32.9%) showed BAI scores > 20, and were considered to be anxious; 26 women (18.6%) presented both depression and anxiety. Women living under more vulnerable social conditions more frequently presented both depression and anxiety, namely younger women, those with lower education levels and lower income, those who did not have private health insurance and women who had more children.
