Evaluating Effectiveness of Mass and Continuous Long-lasting Insecticidal Net Distributions Over Time in Madagascar: A Sentinel Surveillance Based Epidemiological Study

Abstract: Background The reduction of global malaria burden over the past 15 years is much attributed to the expansion of mass distribution campaigns (MDCs) of long-lasting insecticidal nets (LLIN). In Madagascar, two LLIN MDCs were implemented and one district also benefited from a community-based continuous distribution (CB-CD). Malaria incidence dropped but eventually rebounded after a decade. Methods Data from a sentinel surveillance network over the 2009–2015 period was anal… Show more

“…One of the primary goals of the Malawi Malaria Strategic Plan 2017–2022 is to achieve universal LLIN coverage for all households 11 . Research from Madagascar shows that while LLIN mass distribution campaigns may only provide community protection for one year, protection can be sustained when campaigns are followed by continuous LLIN distribution to eligible households, including recently married couples, immigrants, children of vaccination age, and homes with uncovered sleeping areas 47 . Malawi could benefit from education on consistent and correct use of bed nets, and through expansion of continuous LLIN distribution services to additional populations beyond pregnant women, targeting younger individuals living in rural areas with high prevalence of infection for more frequent net replacement.…”

“…That said, we recognize there are other benefits of providing malaria control initiatives even in areas at lower levels of endemicity and that setting the threshold arbitrarily at the median MEI could have dire consequences. In particular, it is important to consider the implications for the duration of the campaign as highlighted by Girond et al [2018] [56]. After about a year the effectiveness of mass distribution campaigns of LLINs seems to decrease while continuous distribution of LLINs maintains a reduction in malaria case notification.…”

“…The level of use of LLINs varies according to several factors, such as temperature, humidity, season, and, especially, the density of mosquitoes, and access to them plays a major determinant of their use [86,87]. LLINs have contributed to the reduction in malaria over the past 15 years, combined with other new control measures, such as IRS and artemisinin-based combination therapies [88] in children and pregnant women [87,89]. Other important advantages of LLINS include reduced consumption of insecticides and insecticide released into the environment because they do not need retreatment [85].…”
