Disorders affecting fetal growth are commonly associated with premature birth. IGFs and their binding proteins (IGFBPs) are potent regulators of fetal growth. In vitro evidence suggests that they regulate collagen turnover. Collagen turnover can be monitored by serum markers of type I collagen synthesis (PINP) and degradation (ICTP) and a marker of type III collagen synthesis (PIIINP). We examined whether these markers in fetal circulation reflect intrauterine growth and maturity, and whether any interrelationship exists between them and fetal IGFs and IGFBPs in preterm infants before 32 wk of gestation. Cord plasma PINP, ICTP, PIIINP, IGF-I, IGF-II, IGFBP-1, and IG-FBP-3 were determined for 98 preterm infants. To express birth weight in units adjusted for gestational age, a birth weight SD score (SDS) was calculated. Negative correlations existed between gestational age and PINP (r ϭ Ϫ0.43; p Ͻ 0.0001), ICTP (r ϭ Ϫ0.34; p ϭ 0.002), and PIIINP (r ϭ Ϫ0.34; p ϭ 0.0001). Positive correlations existed between birth weight SDS and PINP (r ϭ 0.40; p ϭ 0.0002) and ICTP (r ϭ 0.48; p Ͻ 0.0001) but not PIIINP. Moreover, birth weight SDS was positively correlated with IGF-I (r ϭ 0.58; p Ͻ 0.0001) and IGFBP-3 (r ϭ 0.44; p Ͻ 0.0001) and negatively correlated with IGF-II (r ϭ Ϫ0.36; p ϭ 0.003) and IGFBP-1 (r ϭ Ϫ0.50; p Ͻ 0.0001). Gestational age correlated with IGFBP-3 (r ϭ 0.25; p ϭ 0.03). In preeclampsia, IGF-I was lower (p ϭ 0.002) and IGFBP-1 higher (p Ͻ 0.0001), also after adjustment for fetal size. The number of antenatal glucocorticoid treatments was associated with lower ICTP (p ϭ 0.04), higher IGF-I (p ϭ 0.002), lower IGF-II (p ϭ 0.02), lower IGFBP-1 (p ϭ 0.05), and higher IGFBP-3 (p ϭ 0.004), also after adjustment for potential confounders. In multiple regression analysis, the factors significantly associated with PINP (R 2 ϭ 0.47) were gestational age and IGF-I, and those associated with ICTP (R 2 ϭ 0.54) were IGF-I, gestational age, and antenatal glucocorticoid treatment. We conclude that IGF-I may be involved in regulation of type I collagen turnover in the growing fetus. Cord blood PINP and ICTP reflect both fetal growth and maturity and deserve evaluation as potential indicators of postnatal growth velocity in preterm infants, whereas PIIINP reflects fetal maturity. Aim: The aim of this study was to compare the performance of tests based on the detection of insulin-like growth factor binding protein 1 (IGFBP-1) and placental α -microglobulin-1 (PAMG-1) in diagnosing rupture of fetal membranes (ROM) across different patient populations. Methods: A meta-analysis was conducted on prospective observational or cohort studies investigating ROM tests based on the detection of IGFBP-1 and PAMG-1 meeting the following criteria: (1) performance metrics calculated by comparing results to an adequate reference method; (2) sensitivity thresholds of the investigated tests matching those of the currently available tests; (3) study population, as a minimum, included patients between 25 and 37 weeks of gestation. Sensitivities, specificities, and diagnostic odds ratios were calculated. Results: Across all patient populations, the analyzed performance measures of the PAMG-1 test were significantly superior compared with those of the IGFBP-1 test. Of particular clinical relevance, PAMG-1 outperformed IGFBP-1 in the equivocal group, which comprised patients with uncertain rupture of membranes (sensitivity, 96.0 % vs. 73.9 % ; specificity, 98.9 % vs. 77.8 % ; respectively). Conclusions: Compared with its performance in women with known membrane status, the accuracy of the IGFBP-1 test decreases significantly when used on patients whose membrane status is unknown. In this latter clinically relevant population, the PAMG-1 test has higher accuracy than the IGFBP-1 test.

Gereç ve 89±1,45 (%95 GA: 6,86) (duyarlılık %92,3; özgüllük %36,6; pozitif LR 1,46; negatif LR 0,21, duyarlılık %68,1; özgüllük %57,7; pozitif LR 1,61; negatif LR 0,55, sırasıyla), fakat eğrilerin altında kalan alanlar anlamlı değildi (AUC=0,56, %95 GA 0,67; p=0,380, AUC=0,59, %95 GA 0,70; p=0,196 sırasıyla days.
