Evaluation of Remote Stethoscopy for Pediatric Telecardiology

Abstract: Hands-on ES provided reliable and valid screening for congenital heart disease. Tele-ES was highly reliable but had reduced diagnostic validity. Examiner blinding, bandwidth limitations, and artificial restrictions on the remote assistant may have contributed to this reduced performance. As these factors are easily correctable, we regard the ES as a highly promising tool for pediatric telecardiology.

“…Additionally, other disciplines have applied telemedicine strategies in the management of their patients following less impressive reports of its sensitivity in those medical circumstances. Such disciplines include cardiology (deeming 87-94% sensitivity and a negative predictive value of 75% as safe in the detection of pediatric heart disease) [50][51][52] and even the evaluation of diabetic retinopathy, which now utilizes stereoscopic images as the standard for its detection and evaluation by trained readers [53][54][55][56].…”

“…Over the past 2 decades, several investigators have evaluated different hardware and software solutions for remote murmur evaluation. Belmont and colleagues 109,110 used the synchronous approach incorporating both digital and analog systems. They found significant differences in heart sound characterization compared with face-to-face evaluation by a pediatric cardiologist using an analog stethoscope.…”

“…A recent development has been the transmission of heart sounds by e-mail to a cardiology centre where it is interpreted (13,14). Such a system should be of help to the physician faced with problem auscultatory findings, and could prevent unnecessary consultations, electrocardiograms, echocardiograms and patient travel.…”
