Klinika radiologie a nukleární medicíny LF MU a FN Brno, pracoviště Bohunice SouhrnAngiomatóza je termín pro mnohočetné, postupně proliferující hemangiomy (angiodysplazie), postihující více orgánů či tkání současně. Popisujeme 12letý průběh léčby pacienta s mnohočetnými hemangiomy, lokalizovanými v dutině břišní, v retroperitoneu, v trávicí trubici, v mediastinu a také v obratlích. Diagnóza byla stanovena v roce 2005 při probatorní laparotomii, ve věku 28 let. Léčba byla zahájena ihned po stanovení diagnózy interferonem α. Pro jeho nežádoucí účinky (fatigue, anorexie) bylo použití interferonu α limitováno na první rok, pak dávka interferonu byla postupně redukována až byla zcela vysazena. V letech 2006-2011 byl základem léčby talidomid a dočasně i lenalidomid. Do konce roku 2011 byl pacient díky uvedeným lékům stabilizovaný, bez nutnosti opakovaných transfuzí krve. V roce 2012 se opět stav zhoršil a nemocný vyžadoval několik transfuzí v jednom měsíci. Testovali jsme metronomické podávání cyklofosfamidu a dále podávání propranololu, ale ani jeden z těchto léků situaci nemocného nezlepšil. Injekce oktreotidu (Sandostatin 0,1 mg 2krát denně) pomohly snížit ztráty při krvácení do zažívacího traktu. Nicméně pacient byl pořád závislý na transfuzích krve. Proto od roku 2013 byl do léčby přidán bevacizumab (7,5 mg/kg v 3týdenních intervalech). Tato léčba stabilizovala nemocného, snížila spotřebu transfuzí po dobu 2 let, ale po 2 letech úspěšné léčby bevacizumabem byla na CT zobrazení prokázána progrese nemoci a také se objevil hemoragický pleurální výpotek. Po zvládnutí hemoragického výpotku jsme počátkem roku 2015 přešli na podávání afliberceptu, zpočátku v dávce 4 mg/kg ve 14denních intervalech. Vznik masivní proteinurie vedl ke snížení dávky na 2 mg/kg při zachování 14denních intervalů. V této dávce toleruje pacient léčbu afliberceptem bez signifikantních nežádoucích účinků. V době publikace je pacient léčen afliberceptem již 24 měsíců, z toho posledních 10 měsíců byl zcela nezávislý na transfuzích. Těsně před zahájením léčby afliberceptem jeho stav vyžadoval několik transfuzí týdně. Z uvedeného popisu je zřetelné, že účinnost jednotlivých léků je u mnohočetné angiomatózy vždy časově omezená a pro zvládnutí onemocnění je třeba stále nových a účinnějších léků. Bevacizumab a aflibercept jsou přínosem pro pacienty se závažnými formami mnohočetné angiomatózy. Efficacy of Combined Topical Timolol and Oral Propranolol for Treating Infantile Hemangioma: A Meta-Analysis of Randomized Controlled Trials

Background Oral propranolol has become the first-line treatment for infantile hemangioma (IH). However, combined therapy with topical timolol and oral propranolol has been proposed as a more effective IH treatment strategy. We aimed to compare the safety and efficacy of topical timolol, oral propranolol, and their combination for treating IH in a meta-analysis. Methods Relevant randomized controlled trials (RCTs) were obtained after searching the PubMed, Embase, Cochrane’s Library, China National Knowledge Infrastructure, and WanFang databases. A random-effect model was used to pool the results. Results Eight RCTs with 759 patients with IH were included in this meta-analysis. Treatment with topical timolol alone showed a similar response rate compared to oral propranolol (risk ratio [RR] = 0.97, p = 0.63), but resulted in fewer adverse events (RR = 0.36, p = 0.002). Combined treatment with topical timolol and oral propranolol showed a favorable response rate compared to treatment with oral propranolol (RR = 1.14, p = 0.03) or topical timolol (RR = 1.36, p = 0.01) alone. Moreover, combined treatment showed similar risks of adverse events compared to oral propranolol (RR = 0.80, p = 0.24) or topical timolol (RR = 1.31, p = 0.25) alone. Conclusions Combined treatment with topical timolol and oral propranolol may be more effective than either single treatment strategy in patients with IH. Topical timolol alone conferred similar efficacy for IH compared to oral propranolol, but with less incidence of adverse events.

Hemangiomas are benign tumors, which cause an abnormal proliferation of endothelial cells. In recent years, propranolol (POP) has been used for the therapy of infantile hemangiomas (IH). Topical POP has been prescribed instead of systemic steroids to reduce the risk of side effects. In order to increase the effectiveness of the POP, nanoemulsions (NEs) were developed as drug delivery carriers for topical delivery. NEs are stable dispersions consisting of oil, water and surfactants. To develop NEs water‐in‐oil, POP was solubilized in two concentrations, at 0.25 wt% and 0.5 wt% in water. To prepare the external phase, the almond oil, a biocompatible vegetable oil, was chosen due its anti‐inflammatory properties. An optimal proportion of surfactant and co‐surfactant, Tween 80/Span 80, were assessed by pseudo‐ternary phase diagram. NEs were obtained by ultrasonic processor and the mean droplet size was measured by Dynamic Light Scattering (DLS) and it was less than 300 nm. POP was included in the best formulation for stability studies, Nuclear Magnetic Resonance (NMR) studies, by the analysis of proton relaxation time, cell viability studies and occlusivity test. At the end of the work, the stable NEs, containing POP, diluted above 200 fold, did not affect cell viability and it can be considered as a promising formulation for the treatment of superficial infantile hemangioma.
