Background: Radiofrequency has been proven to penetrate deeper than optical light sources independent of skin color allowing a safer treatment for the Asian skin type. Many studies have indicated the efficacy of various types of devices, but have not included a sufficient objective evaluation. Multisource radiofrequency uses multiple phase controlled radiofrequency generators with real time impedance control, allowing painless, deeper dermal heating with better adaptation to differences in individual skin impedance. In this study we used three-dimensional imaging for the objective evaluation of facial skin tightening by multisource phase-controlled radiofrequency. Methods: Twenty Japanese patients were treated with a multisource phase-controlled radiofrequency device. Three-dimensional imaging was performed with a Canfield Scientific Vectra camera and software, and quantitative volume measurements were taken to evaluate the change in the post-treatment volume. The patients then provided subjective assessments. Results: Objective assessments of the treated cheek volume evaluated by a three-dimensional color schematic representation with quantitative volume measurements showed significant improvement three months after the final treatment. The mean volume reduction at the last post-treatment visit was 3.878 ± 2.86 mL. The post-treatment volume was significantly reduced compared to the pretreatment volume in all of the volunteers (P = 0.0007). Ninety-five percent of volunteers reported satisfaction with the improvement of skin laxity, and ninety percent of volunteers reported satisfaction with the improvement of wrinkles, such as the nasolabial folds. Conclusions: The advantages of these multisource phase-controlled radiofrequency treatments are its high efficacy for skin tightening associated with minimal level of discomfort, minimal side effects, and low cost. Taken together, these characteristics facilitate the ability to give repeated treatments as a stand alone treatment or adjunct to surgery. This study provides for the first time a qualitative and quantitative volumetric assessment, proving the ability of the technology to reduce the volume through non invasive skin tightening.

Background: Although monopolar radiofrequency treatment is effective in the improvement of skin laxity, the pain and cost that are associated with this method suggest the need for alternative treatment options. Recently, an infrared device with contact cooling has been shown to be effective in the treatment of skin laxity, with ultrastructural changes observed that are similar to those that are observed following treatment with a monopolar radiofrequency device. However, no control was included in previous studies. Objective: To conduct a prospective, split-face, singleblinded study to look at the efficacy and complications among Asians of treatment for skin tightening with an infrared device with contact cooling. Method: Thirteen Chinese women were treated. An infrared device with contact cooling (Titan, Cutera, Brisbane, CA) was used to treat one side of the face and the untreated side served as the control. The treatment was performed twice with a 4-week interval between the treatments and the patients were followed up by subjective assessment using a structured questionnaire 1 and 3 months after the second (and last) treatment. In all cases, pre-and posttreatment clinical photographs were taken. Two independent observers assessed the photographs. Results: Twenty-three percentage of patients reported mild improvement, 15% reported moderate improvement, and 54% reported significant improvement 3 months after their second (and last) treatment. In terms of objective assessment, 41% of patients were identified to have some degree of improvement of the treated side 3 months after their second treatment. Compared with the untreated side, the treated side improved significantly (P ¼ 0.031) at 1 and 3 months after the second treatment. Blistering occurred in one patient, which had resolved completely by the 3-month follow-up visit. Conclusion: An infrared device with contact cooling can be used effectively and safely for the treatment of skin laxity, especially in smaller anatomical areas.

Comparison of Microneedle Fractional Radiofrequency Therapy with Intradermal Botulinum Toxin A Injection for Periorbital Rejuvenation

Background: For periorbital rejuvenation, injection of botulinum toxin A (BoNT/A) is known to improve both static as well as dynamic wrinkles. A microneedle fractional radiofrequency (MFR) device was recently developed and is a novel and promising tool. Objective: This study compares the effects of these two treatment modalities on periorbital static wrinkles and lines. Methods: Twelve healthy women aged 20-59 years with periorbital wrinkles participated in this study. Each patient received one session of intradermal injection of BoNT/A on the left periorbital area and three sessions of MFR on the right. Clinical improvement, skin elasticity and subjective satisfaction were evaluated at every visit (baseline, 3, 6 and 18 weeks). Results: BoNT/A injection showed superior effects at 3 and 6 weeks. However, the MFR device showed better improvement at 18 weeks. In skin biopsies, the expression of procollagen 3 and elastin was increased on the MFR side compared to the untreated skin and the BoNT/A injection side. The patient satisfaction surveys at 3 weeks showed better satisfaction on the BoNT/A treatment side compared to the MFR treatment side. At 18 weeks, there were no significant differences in patient satisfaction between the two sides. Conclusion: BoNT/A injection rapidly improved periorbital wrinkles, but the effect decreased up to week 18. Compared to BoNT/A injection, MFR therapy showed gradual and long-term improvement in periorbital rejuvenation.
