Evidence-Based Support for Nutrition Therapy in Head and Neck Cancer

Abstract: Purpose of Review Patients diagnosed with head and neck (H&N) cancer often present in a malnourished state for varied reasons; nutritional optimization is therefore critical to the success of treatment for these complex patients. This article aims to review the current nutrition literature pertaining to H&N cancer patients and to present evidencebased strategies for nutritional support specific to this population. Recent Findings Aggressive nutritional intervention is frequently required in the H&N cancer pati… Show more

“…Nutrition is crucial in all stages of the antineoplastic treatment 27 and the treatment may be conditional on the limitations arising from injuries in the oral mucositis cavity. 6,28 These factors can affect the patient's health status in reason of difficult to nutrition before the diagnosis inherent to cancer in development. 29…”

“…11,111,112 Further, cancers of the head and neck receiving surgery and chemoradiation often require enteral or parenteral nutrition to overcome challenges to eating and consuming food. 113 Malabsorption is also a common concern in certain tumour types (e.g. gastro-oesophageal and pancreatic) where tumour location and different treatments may result in a reduced ability to absorb nutrients.…”

“…These findings are probably due to the exacerbation of the obliterative endarteritis and fibrosis induced by the (chemo) radiation itself in local tissues [ 16 ]. While malnutrition should be aggressively corrected during the whole perioperative period [ 17 ], in the setting of a salvage laryngectomy/pharyngolaryngectomy the prophylactic use of a reinforcing flap has been shown to significantly decrease PCF risk [ 18 ]. In addition, while early oral feeding seems to increase PCF risk according to a recent meta-analysis [ 19 ], early oral hydration may actually reduce this probability (by a possible mechanical detersion of infected fluids and saliva on pharyngeal suture) [ 20 ].…”
