Clinicians have managed and treated lower back pain since the earliest days of practice. Historically, lower back pain and its accompanying symptoms of radiating leg pain and muscle weakness have been recognized to be due to any of the various lumbar spine pathologies that lead to the compression of the lumbar nerves at the root, the most common of which is the radiculopathy known as sciatica. More recently, however, with the increased rise in chronic diseases, the importance of differentially diagnosing a similarly presenting pathology, known as lumbosacral plexopathy, cannot be understated. Given the similar clinical presentation of lumbar spine pathologies and lumbosacral plexopathies, it can be difficult to differentiate these two diagnoses in the clinical setting. Resultingly, the inappropriate diagnosis of either pathology can result in ineffective clinical management. Thus, this review aims to aid in the clinical differentiation between lumbar spine pathology and lumbosacral plexopathy. Specifically, this paper delves into spine and plexus anatomy, delineates the clinical assessment of both pathologies, and highlights powerful diagnostic tools in the hopes of bolstering appropriate diagnosis and treatment. Lastly, this review will describe emerging treatment options for both pathologies in the preclinical and clinical realms, with a special emphasis on regenerative nerve therapies.

Peripheral nerve injury (PNI) is a fairly common pathology—PNI accounts for 1-5% of all peacetime injuries and 12% of all combat injuries. This injury leads to disability, the development of chronic pain syndromes and a significant deterioration in the quality of life of the victims. Unfortunately, at present, in the case of the most frequent type of combat trauma — damage to the limbs — PNI treatment is mostly done last, "on the residual principle." Modern means of surgical and conservative treatment of PNI do not provide complete restoration of lost functions, therefore, restorative treatment of PNI is an urgent biomedical problem. The article reviews the currently known molecular mechanisms of various stages of PNI, as well as the plasticity of the central parts of the nervous system on the background of this injury. The main reasons for the limitation of autogenous recovery of functions after a sustained PNI are described — the absence of a relevant spatial organization of regrowth of axons in the area of PNI; post-traumatic death of neurons of spinal nodes and central parts of the nervous system; failure of plastic reconstruction of brain and spinal cord neural networks; irreversibility of atrophy of denervated muscles. Based on this, it was established that the means of restorative treatment of PNI should touch not only the epicenter of PNI, but also the central parts of the nervous system and denervated muscles. Mesenchymal stem cells (MSCs) are well-known means of a positive influence on the the restorative process in the focus of PNI, as well as a source of supportive influence/ strengthening effect and an amplifier of the plasticity of brain neural networks, which makes these cells a promising element of bioengineering treatment of PNI. The effect of MSCs on the central parts of the nervous system in case of PNI remains the least studied. Data from the literature indicate that such an effect can provide support for secondarily affected neurons and stimulate the plastic reorganization of brain networks, i.e., in general, significantly improve the results of restorative treatment of PNI.

Peripheral nerve injury (PNI) resulting in lesions is highly prevalent clinically, but current therapeutic approaches fail to provide satisfactory outcomes in many patients. While peripheral nerves have intrinsic regenerative capacity, the regenerative capabilities of peripheral nerves are often insufficient to restore full functionality. This highlights an unmet need for developing more effective strategies to repair damaged peripheral nerves and improve regenerative success. Consequently, researchers are actively exploring a variety of therapeutic strategies, encompassing the local delivery of trophic factors or bioactive molecules, the design of advanced biomaterials that interact with regenerating axons, and augmentation with nerve guidance conduits or complex prostheses. However, clinical translation of these technologies remains limited, emphasizing the need for continued research on peripheral nerve regeneration modalities that can enhance functional restoration. Experimental models that accurately recapitulate key aspects of peripheral nerve injury and repair biology can accelerate therapeutic development by enabling systematic testing of new techniques. Advancing regenerative therapies for PNI requires bridging the gap between basic science discoveries and clinical application. This review discusses different in vitro models of peripheral nerve injury and repair, including their advantages, limitations, and potential applications.
