Expanding the Pancreas Donor Pool

Abstract: Currently underutilized and in somewhat short supply, the pancreas allograft is a relatively scarce resource. Despite factors such as increased demand, positive changes in organ donation rates, and the liberalization of 'organ-acceptable' criteria, there has been a decline in the number of deceased donor pancreas transplants performed in the US. Concomitantly, there has been an increase in similar transplants performed in other countries. While it is understandable that there are a multiplicity of factors to b… Show more

“…Surprisingly, many human pancreata are discarded after recovery from cadaveric donors, which represents a lost precious resource. Presently in the US, only ~17% of donor pancreata are recovered for transplantation 30 32 and ~25% of those recovered with intent to transplant are discarded 32 . While there are multiple reasons for not recovering pancreata or not transplanting after recovery, numerous organs are available for research through organ procurement organisations.…”

“…The pancreas discard rate remains the highest amongst all abdominal solid organs [5, 18,19]. Despite significant improvement in the outcomes of all types of pancreas transplantation (SPK, PTA, Pancreas After Kidney) [20], the number of pancreas transplants have decreased dramatically in recent years.…”

“…Apart from the overall shortage of cadaveric donor pancreata, the quality of a donor pancreas also plays an important role in determining transplant outcomes, with factors such as donor age and BMI, cause of death, duration of hospital stay, organ procurement team and cold ischemia time acting as major contributors [97][98][99][100][101][102]. Currently, the islet transplantation procedure is hindered by the quality of pancreas allocated.…”
