Experience with FreeStyle Libre Flash glucose monitoring system in management of refractory dumping syndrome in pregnancy shortly after bariatric surgery

Abstract: Bariatric surgery is an effective therapy for obesity but is associated with long-term complications such as dumping syndromes and nutritional deficiencies. We report a case of a 26-year-old caucasian female, with history of morbid obesity and gestational diabetes (GDM), who became pregnant 4 months after Roux-en-Y bypass surgery. She developed GDM during subsequent pregnancy, which was initially managed with metformin and insulin. Nocturnal hypoglycaemia causing sleep disturbance and daytime somnolence occure… Show more

“…In fact, most patients with hypoglycemia after bariatric surgery will respond to dietary modification or pharmacologic intervention . As an example, continuous glucose monitoring was useful in a pregnant patient with dumping syndrome after RYGB and poor adherence with conventional glucose monitoring .…”

“…Anecdotal reports suggest that monitoring glycaemia is useful in patients with suspected dumping syndrome [80][81][82][83][84] . One case report and one therapeutic study used continuous glucose monitoring as an outcome variable to assess the effect of acarbose and dietary measures 75,82 .…”

“…FL has been shown to reduce the time spent in hypoglycaemia by 38% in a group of well controlled patients with T1DM (HbA1c ≤7.5%) and has been well tolerated by its users thus far [ 55 56 ]. It therefore represents an attractive alternative to ‘classic’ CGM systems in certain, but not all clinical scenarios in T1DM management as well as in other indications for CGM use [ 57 ]. Table 2 provides a summary of the main features of currently available devices for BG/IG monitoring.…”
