Experience With Liver And Kidney Allografts From Non-Heart-Beating Donors

Abstract: Given the shortage of cadaveric organs, we began a study utilizing NHBD for OLTx and KTx. There were 24 NHBD between January 1989 and September 1993. These donors were divided into 2 groups: uncontrolled NHBD (G1) (n=14) were patients whose organs were recovered following a period of CPR; and controlled NHBD (G2) (n=10) were patients whose organs were procured after sustaining cardiopulmonary arrest (CA) following extubation in an operating room setting. Eight kidneys and 5 livers were discarded because of mac… Show more

“…To assure the quality of donor livers, it is essential that experienced surgeons carry out the DCD procurement. Casavilla et al (25) described the super rapid technique for DCD procurement. Heaton and colleagues also emphasized the importance of this technique (13).…”

“…The procurement was performed with a modification of the super-rapid technique previously described. 14,15 DWIT was defined as the interval between hypotension with a systolic blood pressure < 50 mm Hg and/or desaturation < 80% (whichever came first) and the initiation of aortic perfusion. CIT was defined as the time between aortic perfusion at organ retrieval and the start of implantation (when the liver was taken out of the ice).…”

“…Kidneys, livers, pancreas and lungs from nonheart-beating donors have been successfully transplanted with good results (205)(206)(207)(208)(209)(210)(211)(212). This can be a substantial additional source of cadaveric organs for transplantation.…”
