Experimental selective choriocapillaris photothrombosis using a modified indocyanine green formulation

Abstract: Restrictions to the use of ICG in aqueous solution, low singlet oxygen quantum yields and high aggregation tendency, were overcome with L-ICG. The lower laser irradiance required to obtain choriocapillaris occlusion may suggest that L-ICG is a more potent and selective photosensitiser than R-ICG.

“…The amphiphilic character of ICG induces aggregation in aqueous solution resulting in adverse effects on its optical properties, such as lower quantum yields due to self quenching (17,18). Also, tumor localization is limited to a rather short period of time after administration due to the strong plasma protein binding of ICG and subsequent rapid clearance by the liver (19).…”

“…Results with this smaller molecular weight compound are promising, showing regression of polypoidal changes (16,17) . A combination of PDT therapy and ranibizumab could provide a good alternative, by reducing the number of injections needed relative to that used in other anti-VEGF monotherapy. There are also reports of phototrombosis of neovessels, mediated by indocyanine green (18) . Cystoid macular edema (CME) can have a variety of causes, including diabetic maculopathy, age-related macular degeneration, retinal vein occlusions, chronic uveitis, epiretinal membranes, choroidal tumors, radiation retinopathy, perifoveal retinal telangiectasis, retinitis pigmentosa, dominantly inherited CME, foveal X-linked retinoschisis, and others.…”

“…The final mean improved by 7.2 and 11.3 letters in the Marina (11) and Anchor (12) trials, respectively. However, other treatments have been used, including laser photocoagulation (15,16) , verteporfin photodynamic therapy (PDT) (17) , photothrombosis with indocyanine green (18) , and pegaptanib sodium intravitreal injection (19) . This report is a revision paper and investigated the results of ranibizumab intravitreal monotherapy in the treatment of CNV in the context of AMD with extensive pre-existing GA.…”
