Exploratory Factor Analysis of the Beck Anxiety Inventory and the Beck Depression Inventory-II in a Psychiatric Outpatient Population

Abstract: BackgroundTo further understand the relationship between anxiety and depression, this study examined the factor structure of the combined items from two validated measures for anxiety and depression.MethodsThe participants were 406 patients with mixed psychiatric diagnoses including anxiety and depressive disorders from a psychiatric outpatient unit at a university-affiliated medical center. Responses of the Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI)-II, and Symptom Checklist-90-Revised (SCL… Show more

“…The symptoms are rated on a 4-point Likert scale, ranging from 0 (‘not at all’) to 3 (‘severely’). A cut-off value of 21 for the BAI identified the participants with more than moderate anxiety symptoms [ 29 , 30 ].…”

“…This opposing effect is indeed also observable in the original data: ρ = −0.32 , p = 0.07 for “loss of pleasure” and “guilty feelings” and ρ = −0.35, p < 0.05 for “irritability” and “concentration difficulty” (Spearman correlation, uncorrected). Several authors have performed exploratory [e.g., Brown et al ( 2012 ), Lee et al ( 2018 )] and confirmatory [e.g., Morley et al ( 2002 ), Tobias et al ( 2017 )] FA on BDI data of different patient populations with varying methods and results. In their work, Brown et al ( 2012 ) report only positive correlations between the single BDI items in chronic fatigue patients using a two-factor model accounting for 35.7% of the overall variance.…”

“…In their work, Brown et al ( 2012 ) report only positive correlations between the single BDI items in chronic fatigue patients using a two-factor model accounting for 35.7% of the overall variance. Lee et al ( 2018 ) created a combined model of Beck's anxiety and depression inventories using a five-factor model (56.2% of combined variance explained) for a mixed group of psychiatric patients. Since those studies used either oblique rotation (allowing for correlation between factors, which was unwanted here) or did not further specify it, direct comparisons of the factors are difficult due to differently oriented coordinate systems.…”

“…Motor phenotypes including tremor‐dominant and non‐tremor dominant type were additionally assessed according to a previous report . For assessments of NMS, the Korean version of the Montreal Cognitive Assessment (MoCA‐K), the Beck Depression Inventory, the Beck Anxiety Inventory and the Parkinson Fatigue Scale were used. In our movement disorder clinic, each patient with PD was routinely asked the following question about dizziness: “Do you have dizziness?” If the patient said “yes”, then detailed characteristics of dizziness, including frequency, duration, and nature, were additionally evaluated.…”
