Extracorporeal Membrane Oxygenation Use Has Increased by 433% in Adults in the United States from 2006 to 2011

Abstract: Recent studies have shown the benefits of extracorporeal membrane oxygenation (ECMO) in supporting adults with severe respiratory or cardiac failure refractory to conventional treatments. The purpose of this investigation was to analyze the usage of ECMO in adults to identify recent trends within the United States. The usage of ECMO, the survival rates, and the hospitalization costs from 2006 to 2011 were analyzed using the Nationwide Inpatient Sample from the Healthcare Cost and Utilization Project. The rate … Show more

“…Significant improvements in the ECMO circuit allowed for more aggressive ECMO use during the H1N1 influenza epidemic in 2009 (3). As a consequence, there has been sustained interest in adult ECMO, with an over 400% increase in U.S. cases from 2006 to 2011 (4,5).…”

“…Less subject to debate, however, is the observation that patients who require ECMO likely have substantially increased healthcare costs relative to conventional management (4). Importantly, the number of hospitals offering ECMO has grown rapidly, particularly for adult patients (10,11), raising the concern that rapid diffusion of low-volume advanced care modalities across multiple institutions might compromise their efficiency and effectiveness (12,13).…”

“…These findings are particularly relevant as there has been a 433% increase in the number of adult patients supported with ECMO in the United States since 2006. 19 Interestingly, respondents from ACC reported to be more knowledgeable regarding ECMO compared with other specialties, suggesting that there is a gap in critical care training regarding ECMO exposure. A previous survey conducted among critical care physicians (in Australia and New Zealand) demonstrated lack of training and availability as a factor in limiting the ECMO use in ARDS.…”
