Factors Associated With Hepatitis B Vaccination Among Men Having Sexual Relations With Men in Montreal, Quebec, Canada

Abstract: A significant proportion of Montreal's MASM, some of whom are at risk of contracting HBV through sexual and parenteral transmission, have not been vaccinated for HBV. Men who have affective and sexual relations with men should be educated about the risk of HBV transmission and the seriousness of the disease.

“…For this reason, we have analyzed our data specifically to identify factors associated with failure to initiate and complete HBV vaccination. 25 Our finding of a strong association of HBV infection with STD reinforces the importance of timely diagnosis and treatment of STDs and, in particular, genito-ulcerative infections. This will also help to prevent HIV infection.…”

“…This strategy has several limitations. Risk factors cannot be identified for about 30% of HBV cases (9); there is poor compliance to the HBV vaccine schedule in high risk groups (40,41); and HBV infections occur at early stages of intravenous drug use before individuals are identified as being at risk (31).…”

“…An estimated 6,000 deaths occur annually from these complications per year (Koff, 1999;Schafer & Sorrell, 1999). The U.S. CDC recommend vaccination against HBV for all men who have sex with men (MSM), whether self-identifying as gay, bisexual, or not, (CDC, 1991) a group among whom HBV has been identified as one of the most important sexually transmitted diseases (STD; CDC, 1991;Dufour et al, 1999; Institute of Medicine & Committee on Prevention and Control of Sexually Transmitted Diseases, 1997; Kane, 1995;Kingsley et al, 1990). However, available national epidemiologic data and results from community-based HBV vaccination programs indicate that the majority of gay men are not vaccinated (Institute of Medicine & Committee on Prevention and Control of Sexually Transmitted Diseases, 1997;Kane, 1995;MacKellar et al, 2001).…”

“…Estimates of the prevalence of previous hepatitis B virus infection in this population range from 5 to 81%; prevalence of HBV surface antigen vary from 1 to 11% (Brook, 1998;Seage et al, 1997). Self-reported vaccination rates among sexually active self-identifying and non-self-identifying gay men are low, ranging from 11 to 48% (Dufour et al, 1999;Kane, 1995;Katz, 1996;Loke, Murray-Lyon, Balachandran, & Evans, 1989;Neighbors, Oraka, Shih, & Lurie, 1999;Rhodes, DiClemente, Yee, & Hergenrather, 2000, 2001aSeage et al, 1997). Unfortunately, little is known about HBV vaccination acceptance among these men that could help guide intervention efforts (Dufour et al, 1999;MacKellar et al, 2001;Neighbors et al, 1999;Rhodes et al, 2000;Rhodes et al, 2001a;Seage et al, 1997;Yuan & Robinson, 1994), and no published scales exist to measure possible latent variable correlates of HBV vaccination among gay men (Rudy Detels, Douglas, Greenland, 2003;Yee and Rhodes, 2002).…”
