Factors associated with stroke among adult patients with hypertension in Ayder Comprehensive Specialized Hospital, Tigray, Ethiopia, 2018: A case-control study

Abstract: Background Globally, the burden of stroke is increasing at an alarming rate. Factors associated with stroke among hypertensive patients are not consistent across different studies and there are limited studies particularly to hypertensive stroke in the particular setting. This study aimed to assess factors associated with stroke among patients with hypertension in Ayder Comprehensive Specialized Hospital, Mekelle, Tigray, Ethiopia, in 2018. “…Upon multivariable backward logistic regression, those study participants with a history of substance abuse had almost three times higher (AOR=2.839, P=0.024) probability of poor treatment outcome than those who had not. This finding was in line with the studies in northern Ethiopia 29,42 that reported the significant association of social history with poor treatment outcome. Besides, those patients who did not receive stroke medication during admission had at least twelve times higher likely hood of poor treatment outcomes than their counterparts (AOR=12.503, P≤0.001).…”

“…Our study found that dyslipidemia, atrial fibrillation, lack of exercise, and family history of stroke were independent influencing factors for stroke in hypertensive population. Lack of exercise and dyslipidemia have also been shown to be significantly related to the prevalence of stroke in hypertensive patients in some studies, which is consistent with our findings [ 19 , 20 ]. In addition, several studies have reported that alcohol consumption, excessive salt intake, uncontrolled blood pressure, high carotid artery diameter and plaque may all be risk factors for stroke in hypertensive population [ 19 22 ].…”

“…The number of participants in each study ranged from 73 to 503. The most retrieved studies (n = 8) were from Oromia [ 29 36 ] followed by Addis Ababa (n = 7) [ 37 – 43 ], Amhara (n = 7) [ 44 50 ], Tigray region (n = 4) [ 51 54 ], and Southern Nations Nationalities and People’s (SNNP) (n = 1) [ 55 ].…”

“…Out of 27 studies, 21 (77.78%) studies received a low risk of bias. 5 studies [ 29 – 31 , 36 , 39 , 44 , 49 , 51 , 55 ] had a high risk of case definition, five studies [ 29 , 30 , 35 , 45 , 55 ] had random selection bias, and 14 studies [ 29 , 33 35 , 39 , 41 , 46 , 47 , 51 , 53 , 56 ] had a high risk of representation bias ( S2 Table ).…”
