Factors influencing response to ingenol mebutate therapy for actinic keratosis of face and scalp

Abstract: AIMTo determine factors independently influencing response to ingenol mebutate therapy and assess efficacy on clinical setting of non-hypertrophic non-hyperkeratotic actinic keratosis (AK).METHODSConsecutive patients affected by non-hypertrophic non-hyperkeratotic AKs of the face or scalp were enrolled to receive ingenol mebutate 0.015% gel on a selected skin area of 25 cm2 for 3 consecutive days. Local skin reactions were calculated at each follow up visit using a validated composite score. Efficacy was evalu… Show more

“…Interestingly, the lesion complete response rate for face (mainly forehead) (65%) was slightly higher than for the scalp (58%), possibly because self‐application is more difficult for the scalp than the face. However, a better complete response for AK on the face compared to scalp was also previously observed when MAL DL‐PDT was applied in a clinical setting without self‐application, and also with other AK treatments, such as 5‐fluorouracil and ingenol mebutate …”

“…However, a better complete response for AK on the face compared to scalp was also previously observed when MAL DL-PDT was applied in a clinical setting without selfapplication, 19,20 and also with other AK treatments, such as 5fluorouracil 21 and ingenol mebutate. 22,23 As in the previous randomized controlled trial performed in Europe, 10 the most frequent related adverse events (mostly mild erythema, pain and skin burning sensation) were associated with the PDT mode of action. Although mild procedural pain was reported for 30% of patients, some were very mild pain since 86% of patients indicated a score of ≤2 on a 0-10 NRS scale.…”

“…Considering reported data, ingenol mebutate treatment seems to be more effective on the face than on the scalp and the efficacy seems to be directly related to the level of LSR. 50 …”
