Far Lateral Tubular Decompression: A Case Series Studying One and Two Year Outcomes with Predictors of Failure

Abstract: IntroductionThe optimal surgical treatment of isolated lumbar foraminal stenosis has not been defined. Minimally invasive decompression of the foramen from a far lateral tubular decompression (FLTD) approach has been shown to not only have minimal morbidity but also highly variable success rates at short-term follow-up. It is important to quantify improvement and define the demographic and radiographic parameters that predict failure in this promising, minimally invasive surgical technique. This study investig… Show more

“…LTM techniques utilized were the unilateral laminotomy (UL) to address central and ipsilateral lateral recess stenosis, the unilateral laminotomy for bilateral decompression (ULBD) to address central pathology, bilateral lateral recess pathology and contralateral foraminal stenosis, and the far lateral “transpedicular” decompression (FLD) to address ipsilateral foraminal stenosis. All procedures were performed by a single surgeon using previously described techniques 6,7,17,18…”

“…For the FLD, "scotty dog view" fluoroscopy was used to approach the neuroforamen from an oblique approach, sometimes up to 9 cm off midline, as previously described. 7 This oblique approach permitted decompression of the foramen medial to the pedicle with minimal resection of the pars interarticularis. Oblique fluoroscopy was used to dock the METRx tubular retractor (Medtronic, Dublin, Ireland) to the superior articular process.…”

“… 6 ) This strategy has been modified further, 7 - 9 ) and endoscope-assisted tubular surgeries have also been performed instead of microsurgery. 10 ) For decompression alone, it is important to perform bone removal to a sufficient extent; however, the occurrence of secondary instability is a factor that has to be considered seriously. In view of the instability, lumbar interbody fusion (LIF) has been selected primarily for the treatment of L5/S1 LFS by many spinal surgeons.…”
